胃镜检查在小儿幽门螺杆菌感染相关性胃炎诊断中的价值

Abstract Objective To analyze the diagnostic value of gastroscopy for Helicobacter pylori(Hp)infection associated gastritis in children.Methods A total of 156 patients with suspected pediatric gastritis admitted to our hospital from January to August in 2018 were selected as the study subjects.According to whether the patient agreed to use gastroscopy and the results of the examination were randomly divided into group A(66 cases),group B(14 cases),group C(76 cases).Group A agreed to use gastroscopy and positive for Hp,group B agreed to use gastroscopy and negative for Hp,group C did not agree to use gastroscopy.All three groups were given routine treatment of gastritis,such as namely health education,life restraint,at the same time,acid inhibitors,antibiotics and gastric mucosal protective drugs were given,and gastrointestinal motility drugs were added when necessary.In addition,the children in group A were treated with anti-Hp therapy according to the clinical antibiotics,proton pump inhibitors(PPI)and bismuth agents.The children with severe Hp infection were treated with Amoxicillin,Clarithromycin,Omeprazole and Bismuth Potassium Citrate.The course of treatment was 2 weeks.After one month of treatment,the clinical manifestations and signs of the three groups were compared,and the therapeutic effect was evaluated by pathological examination.Results There was no significant difference in the inflammation and activity of gastric mucosa between group A and group B before treatment(P＞0.05).The severity degree of inflammation in group A was significantly lower than that in group B after treatment,the difference was statistically significant(P＜0.05).The degree of abdominal pain and tenderness in group A was significantly lower than that in group C,the difference was statistically significant(P＜0.05).The degree of abdominal pain and tenderness in group B was significantly lower than that in group C,the difference was statistically significant(P＜0.05).There was no significant difference in the degree of vomiting and anorexia among the three groups(P＞0.05).Conclusion Gastroscopic examination can identify the specific causes of gastritis in children,and it has important diagnostic value for clinical treatment of pediatric gastritis.
