家庭关怀度对产后抑郁症患者心理韧性与抑郁的影响

Abstract Objective To explore the effect of family care degree on psychological resilience and depression in the puerpera with postpartum depression.Methods A total of 2000 puerpera in the department of obstetrics of our hospital from January to December 2017 were selected as objects.The Edinburgh postnatal depression screening scale(EPDS),family care index(APGAR),the Connor-avidson resilience scale(CD-RISC),and Hamilton depression scale(HAMD)were used to screen postpartum depression women and non-postpartum depression women.The differences between family care,psychological resilience and depression were compared between two groups.Pearson correlation analysis was used to analyze the correlation between family care,psychological resilience and depression.The mediating effect analysis method was used to analyze the mediating effect of family care.Results Among 2000 puerperas,the puerpera with postpartum depression were 286,whose detection rate was 14.3%,and the puerpera without postpartum depression were 1714.The APGAR score ([4.64±1.15]points)and CD-RISC total score ([52.83±7.37]points)in postpartum depression women were significantly lower than those in non-postpartum depression women([8.16±1.49]points,[83.56±8.42]points),HAMD total score ([19.35±4.29]points)was significantly higher than that in non-postpartum depression women([6.29±1.51]points),and the differences were statistically significant(P＜0.01).The results of correlation analysis showed that psychological resilience was positively correlated with family care(r=0.514,P＜0.01),psychological resilience was negatively correlated with depression (r=-0.622,P＜0.01),and family care was negatively correlated with depression (r=-0.638,P＜0.01).The results of regression analysis showed that psychological resilience and family care had significant negative predictive effect on depression (β=-0.894,P＜0.01;β=-0.628,P＜0.01).The value of mediating effect from psychological resilience to depression was 0.263,and the ratio of mediating effect to total effect was 0.294.Conclusion Family care degree plays a partial mediating role between psychological resilience and depression.To improve family care degree helps to promote the level of psychological resilience and improves depression condition.
