右美托咪啶对重度烧伤患者肾功能的影响

Abstract Objective To explore the influence of Dexmedetomidine on renal function in patients with severe burn.Methods Seventy-two patients with severe burn treated in our hospital from January 2017 to January 2018 were selected as subjects,and they were divided into the two groups according to the random number table method,each group had 36 cases.Patients in the study group were given Dexmedetomidine for intravenous pump during escharectomy and dermatoplasty,while patients in the control group were given normal saline of equal volume for or intravenous pump during escharectomy and dermatoplasty.The concentration of serum creatinine,urea nitrogen and serum cystatin C were compared between the two groups at different time points.Results There was no significant difference in serum creatinine concentration between the two groups at 10 min before anesthesia induction (t1)(P>0.05).The serum creatinine concentration in the study group was significantly lower than that in the control group at 30 min after escharectomy and dermatoplasty(t2),10 min before the end of the operation(t3),24 h after operation(t4)and 72 h after operation(t5),and the differences were statistical significant(P<0.05).There was no significant difference in the concentration of urea nitrogen between the two groups at t1(P>0.05).At t2,t3,t4and t5,the concentration of urea nitrogen in the study group was significantly lower than that in the control group,and the differences were statistical significant(P<0.05).There was no significant difference in serum cystatin C concentration between the two groups at t1(P>0.05).At t2,t3,t4and t5,the concentration of serum cystatin C in the study group was significantly lower than that in the control group,and the differences were statistical significant(P<0.05).Conclusion The application of Dexmedetomidine during operation can protect the renal function of patients with severe burn,and it is metabolized through the liver.The clearance rate does not depend on the renal function,so Dexmedetomidine is an extremely ideal anesthetic drug for patients with severe burn.
