前列地尔对重症急性胰腺炎患者早期促炎因子的影响及疗效分析

Abstract Objective To investigate the effect and efficacy of Alprostadil on early proinflammatory factors in patients with severe acute pancreatitis.Methods Sixty-six patients with severe acute pancreatitis who were admitted to our hospital from September 2016 to December 2017 were selected as the study subjects.They were divided into the control group (33 cases)and the observation group (33 cases)according to a double-blind method.The control group was treated with sodium chloride or glucose solution,and the observation group was given Alprostadil.The coagulation function,inflammatory factors,and symptom improvement were compared between two groups.Results There were no significant differences in the levels of platelet distribution width(PDW)and Mean platelet volume(MPV)before treatment between two groups(P>0.05).The levels of PDW and MPV in two groups after treatment were lower than those before treatment,and the differences were statistically significant(P<0.05).The levels of PDW and MPV after treatment in the observation group([12.21±1.05]%,[8.15±0.64]fl)were lower than those in the control group([15.54±1.31]%,[10.28±0.36]fl),and the differences were statistically significant(P<0.05).There were no significant differences in the levels of IL-1β,IL-6,TNF-α before treatment between two groups(P>0.05).The levels of IL-1β,IL-6,TNF-α in two groups after treatment were lower than those before treatment,and the differences were statistically significant(P<0.05).The levels of IL-1β ([32.63±7.4]ng/L),IL-6([61.05±11.94]ng/L),TNF-α ([62.26±17.18]ng/L)after treatment in the observation group were lower than those in the control group([75.59±11.05],[76.69±10.28],[78.58±12.05]ng/L),and the differences were statistically significant(P<0.05).The recovery time of blood amylase ([4.10±1.25]d),intestinal function ([3.18±1.04]d)and the relief time of abdominal pain ([3.15±1.19]d)in the observation group were shorter than those in the control group （[6.65±1.28],[5.37±1.23],[5.01±1.12]d）,and the differences were statistically significant(P<0.05).Conclusion In the treatment of severe acute pancreatitis,the application of Alprostadil can reduce the degree of inflammatory reaction in patients and promote the early disappearance of various symptoms,with reliable effect.
