无创正压通气治疗老年慢性阻塞性肺疾病合并重症呼吸衰竭的临床效果

Abstract Objective To evaluate the clinical efficacy of noninvasive positive pressure ventilation(NIPPV)in the treatment of chronic obstructive pulmonary disease(COPD)complicated with severe respiratory failure in elderly patients.Methods A total of 94 elderly COPD patients with severe respiratory failure were selected from January 2014 to January 2016 treated in our hospital,and divided into control group and observation group according to random number table method,47 cases in each group.Patients in the experimental group were treated with NIPPV and patients in the control group were treated with conventional therapy.The PaO2,PaCO2,SpO2,HR and RR levels were observed before and after treatment,the complications were recorded and the clinical efficacy was evaluated.Results After one course of treatment,HR,RR,PaCO2,SpO2levels in the experimental group were lower than those in the reference group,and PaO2 levels in the reference group were lower than those in the experimental group,the differences were statistically significant(P＜0.05).The clinical effective rate of the experimental group was 87.23%(41/47)which was higher than that in the control group of 74.47% (35/47),the difference was statistically significant(P＜0.05).The incidence of complications in the experimental group was lower than that in the control group,and the difference was statistically significant(P＜0.05).Conclusion The clinical treatment of elderly COPD with severe respiratory failure is effective,reducing the trachea intubation rate and complications,and can improve the prognosis and improve the clinical efficacy.
