慢性阻塞性肺疾病合并呼吸衰竭患者经无创呼吸机治疗的临床效果观察

Abstract Objective To observe the c1inica1 effects of noninvasive venti1ator in treating chronic obstructive pu1monary disease with respiratory fai1ure.Methods 112 patients with chronic obstructive pu1monary disease with respiratory fai1-ure from January 2012 to December 2014 of our hospita1 were random1y se1ected and were random1y divided into contro1 group（n＝56,They were given routine treatment）and observation group（n＝56,They were given noninvasive venti1ator treatment）according to their admitted order.C1inica1 effects,b1ood gas and changes of c1inica1 symptoms before and after treatment in two groups were eva1uated.The incidence of comp1ications were counted.Results①The tota1 effective rate in observation group was 91.07％（n＝51）,obvious higher than that was 67.87％in contro1 group（n＝38）（P＜0.05）.The comp1ication rate of the observation group was 3.57％（n＝2）,which was significant1y 1ower than 28.57％in the contro1 group （n＝16）.Both of tota1 effective rate and incidence rate of comp1ications between two groups had obvious difference,and the difference was statistica1 significance（P＜0.05）.②After treatment,a11 of PaCO2,PaO2,SaO2,RR and HR in two groups was improved obvious respective1y,and the index in observation group was better than that of contro1 group respective1y.The comparison in the group of a11 index before and after treatment and inter-group difference after treatment had statistica1 significance（P＜0.05）.Conclusion Using noninvasive venti1ator for chronic obstructive pu1monary disease with respiratory fai1ure patients has obvious c1inica1 effects.It can effective prevent the incidence of comp1ications,re1ief c1inica1 symptoms speed and improve b1ood gas index.It has higher c1inica1 app1ication va1ue.
