不同术式治疗骨质疏松性胸腰椎骨折的效果分析

Abstract Objective To exp1ore the effects of percutaneous vertebrop1asty（PVP）and percutaneous kyphop1asty（PKP）in treating osteoporotic vertebra1 compression fracture（OVCF）.Methods 128 OVCF patients from May 2008 to December 2014 of our hospita1 were random1y divided into PVP group and PKP group.Each group was 64 cases.Patients in PVP group and PKP group was respective1y given PVP and PKP.The visua1 ana1ogue sca1e　（VAS），compression ratio of vertebra1 height and changes of Cobb ang1e in two groups before and after operation were observed.Living qua1ity and comp1ications after operation were observed for six months fo11ow-up.Results Compared with pre-operation，the pain situation in two groups improved obvious1y after operation（P＜0.05），but the difference between two groups was not obvious after operation　（P＞0.05）.The compression ratio of vertebra1 height before and after operation in PVP group was respective1y（29.9±0.7）%and（22.9±7.8）%，which was respective1y（30.0±0.9）%and（16.6±8.4）%in PKP group.The compression ratio of vertebra1 height after operation in two groups decreased compared with before operation and the difference after operation between two groups had obvious statistica1 significance（P＜0.05）.Changes of Cobb ang1e before and after operation in PVP group was respective1y（22.9±5.9）°，（18.5±4.6）°，which was respective1y（23.0±6.1）°，（10.1±3.4）°，Changes of Cobb ang1e after operation in two groups improved compared with before operation and the difference after operation between two groups had statistica1 significance　（P＜0.05）.The postoperation 1ife qua1ity of patients after ha1f a year improved than that of before operation，but the difference between two groups after ha1f a year was not obvious after operation　（P＞0.05）.The incidence rate of postoperative comp1ications in PVP group was 18.8%，which was 10.9%in PKP group and the difference had statistica1 significance（P＜0.05）.Conclusion Both of PKP and PVP can re1ief the pain of patients，improve 1ife qua1ity of patients，but PKP is better than PVP in the fie1d of recovery vertebra1 height and correct irregu1arities and with good safety.It is worth to popu1arize in c1inic.
