二级胸痛中心管理模式对救治急性ST抬高型心肌梗死患者的临床价值

Abstract Objective To exp1ore the c1inica1 app1ication va1ue of centra1 management mode in second-1eve1 chest pain on rescuing patients with ST-segment e1evation myocardia1 infarction　（STEMI）.Methods From January 2007 to December 2009，c1inica1 data from 60 patients with STEMI for emergency admitted into our hospita1 were retrospective1y ana1yzed.The c1inica1 data from another group of 58 patients with STEMI who were admitted into our hospita1 for emergency or transferred into our hospita1 ranging from January 2010 to December 2013 were se1ected as contro1.The centra1 management mode in second-1eve1 chest pain was estab1ished in our hospita1 in 2010.Patients admitted into the hospita1 before estab1ishment were c1assified into contro1 group，and after estab1ishment，these participants were categorized into observation group.The indexes of morta1ity rate，number of patients by percutaneous coronary intervention（PCI），content of brain natriuretic peptide（BNP）7 days after therapy，ejection fraction（EF），incidence of cardiogenic shock，incidence of ma1ignant abnorma1ities of heart rate，reperfusion success rate，and time of door-to-ba11oon were compared. Results The morta1ity，incidence of cardiogenic shock，and incidence of ma1ignant abnorma1ities of heart rate in the observation group were a11 great1y 1ower than those of contro1 group.The proportion of imp1ementing PCI and reperfusion success rate in the observation group were higher than those of contro1 group with statistica1 differences（P＜0.05）.In the observation group，indexes of BNP 7 days after therapy and EF were superior to those of contro1 group.The time of doorto-ba11oon in the observation group was shorter in comparison with that in the contro1 group，which was disp1ayed a statistica1 difference　（P＜0.05）.Conclusion C1inica1 app1ication of centra1 management mode in second-1eve1 chest pain can obtain a remarkab1e effect on rescuing STEMI patients，effective1y shorten therapeutic time of reperfusion in STEMI，decrease morta1ity，and improve patient＇s prognosis.It is worthy of promotion in 1oca1 areas，economic 1ess-deve1oped areas or remote areas.
