Abstract Objective To explore the effect of chronic disease health care model in elderly patients with diabetes mellitus(DM).Methods From January 2020 to January 2021,150 elderly patients with DM who were admitted to Zhongshan Hospital,Xiamen University were selected as the research objects.They were divided into control group (75 cases) and observation group (75 cases) by random number table method.The control group was under routine management,and the observation group was under the management of chronic disease health care model,with continuous intervention for 1 year.The awareness rate of DM related knowledge and glucose metabolism indexes were compared between the two groups.Results Before intervention,there was no significant difference in the awareness rate of DM related knowledge between the two groups (P>0.05).After intervention,the awareness rate of DM related knowledge in the observation group was higher than that in the control group,the difference was statistically significant (P <0.05).Before intervention,there were no significant differences in fasting plasma glucose(FPG)and 2 h postprandial blood glucose(2 h PG)between the two groups(P>0.05).After intervention,FPG and 2 h PG in the observation group were lower than those in the control group,the differences were statistically significant (P<0.05).Conclusion Through the management of chronic disease health care model,elderly DM patients can effectively improve their awareness of disease related knowledge,improve glucose metabolism,and have high clinical application value.
