Abstract Objective To explore the diagnostic value of spectral CT in responsible blood vessels related to hemoptysis. Methods The clinical data of 59 patients with hemoptysis who underwent interventional embolization in First People's Hospital of Fuzhou from June 2020 to December 2021 were retrospectively analyzed. All patients underwent digital subtraction angiography (DSA) and energy spectral CT examinations, and the DSA examination results were used as the "gold standard" to calculate the sensitivity, specificity and accuracy of energy spectral CT in detecting hemoptysis-related responsible blood vessels, and to analyze the correlation between hemoptysis-related responsible blood vessel energy spectral CT and hemoptysis-related responsible blood vessels. DSA imaging features. Results By DSA examination, a total of 124 responsible vessels were detected in 59 patients with hemoptysis, including 82 bronchial arteries (66.13%) and 42 non-bronchial arteries (33.87%). With DSA examination results as the gold standard, the accuracy, specificity and sensitivity of energy spectral CT in detecting responsible vessels related to bronchial arterial hemoptysis were 93.79%, 93.68% and 81.46%, respectively. The accuracy, specificity and sensitivity of spectral CT in detecting responsible vessels related to non-bronchial arterial hemoptysis were 91.53%, 97.23% and 57.14%, respectively. DSA of responsible vessels associated with hemoptysis showed 12 direct signs (extravasation and retention of contrast agent) and 185 indirect signs. There were 9 direct signs (75.00%) and 170 indirect signs (91.89%) of responsible vessels associated with hemoptysis. Conclusion Spectral CT has a high diagnostic efficiency for hemoptysis-related responsible blood vessels, and can clearly display the origin, quantity and imaging characteristics of related responsible blood vessels.
