社区药学服务站开展慢性病患者药物治疗管理的服务实践

Abstract Objective To explore the role and practice of setting up pharmaceutical service station in communities to provide medicine therapy management (MTM) service mode for patients with chronic diseases.Methods From March 2017 to March 2018,MTM service was provided for 75 patients with chronic diseases in a community of Suqian City by the clinical pharmacists.The characteristics of medication-related problems were analyzed before service (from March to August 2017).The improvement of medication-related problems and the evaluation on drug therapy effect by patients were compared and analyzed after service (from September 2017 to March 2018)..Results Through the half-year MTM service,the proportion of patients with irrational drug use decreased from 81.3% to 24.0%.Seven categories of medication-related problems results showed that the proportion of unnecessary drug therapy decreased from 30.7% to 6.7%,the proportion of ineffective drug therapy decreased from 18.7% to 4.0%,the proportion of needs for additional drug therapy decreased from 16.0% to 9.3%,the proportion of dosage too low decreased from 20.0% to 8.0%,the proportion of dosage too high dropped from 6.7% to 0.0%,the proportion of adverse drug events (ADE) dropped from 32.0% to 9.3%,and the proportion of low compliance dropped from 61.3% to 28.0%.After service,the score of Morisky scale in patients ([6.8±1.2] points) were higher than that before service ([4.3±0.8] points),the difference was statistically significant (P<0.05).After service,the total effective rate of the evaluation on the drug therapy effect by the patients was 89.3%,which was higher than that before service accounting for 40.0%,and the difference was statistically significant(P<0.05).Conclusion The implementation of MTM services in a community provides a guarantee for the management of medications for patients with chronic diseases,as well as a reference for the transformation of clinical pharmacists′ work model and the practice of home pharmaceutical care.
