Abstract Objective To explore the classification of pathological changes of chronic atrophic gastritis (CAG) with intestinal metaplasia (IM) and the distribution characteristics of traditional Chinese medicine syndrome.Methods A total of 138 patients with CAG and IM admitted to the Third People′s Hospital of Fujian University of Traditional Chinese Medicine from September 2018 to March 2020 were selected as the research objects.The patient′s gastroscopy,pathological examination and traditional Chinese medicine syndrome elements (position and disease nature) were analyzed and studied..Results In this study,the research data were mainly mild and moderate IM; all patients had gland atrophy,and the gland atrophy was aggravated with IM (P<0.05).Chronic inflammation of the gastric mucosa was polarized,and there was no obvious correlation between the grade of IM and pathological inflammation.The pathology was mainly inactive in the cases of mild,moderate and severe IM.The Helicobacter pylori (Hp) infection rate was low in mild,moderate and severe IM.The comparison showed that the statistically significant difference was found in the distribution of Hp infection of the IM grade (P<0.05),but the detection rate of Hp infection in mild IM cases was higher.The liver,stomach,kidney and spleen were found to be the main disease position.Qi deficiency,yin deficiency,qi stagnation,phlegm,blood deficiency,yang deficiency,dampness,heat,and blood stasis were found to be the main nature of disease.Conclusion Glandular atrophy gradually worsens with the degree of IM.The amount of Hp bacteria in the IM area decrease with the degree of IM,suggesting that endoscopic detection of Hp bacteria should avoid IM area.Disease positions are mainly located in liver,stomach,spleen,and kidney.Natures of disease are mainly distributed in qi deficiency,yin deficiency,qi stagnation,phlegm,blood deficiency,yang deficiency,dampness,heat,blood stasis,etc.
