Application of dual-phase CT angiography of pulmonary artery and bronchial artery in the diagnosis of hemoptysis |

Abstract Objective To explore the application value of pulmonary artery and bronchial artery dual-phase multi-slice spiral CT angiography (CTA) in the interventional embolization treatment of hemoptysis.Methods A total of 62 patients who underwent interventional embolization for hemoptysis in Shangrao People′s Hospital were selected from March 2017 to March 2019.All patients underwent dual-phase CTA examination of pulmonary artery and bronchial artery,and the results of digital subtraction angiography (DSA) were used as the reference to evaluate the conformity of CTA to the detection of diseased vascular signs and the number of responsible blood vessels for hemoptysis,and analyze the CTA imaging manifestations of hemoptysis.Results The coincidence rate of dual-phase CTA for the detection of diseased vascular signs was 81.14%.The coincidence rates of bronchial artery-pulmonary artery malformation and pulmonary pseudoaneurysm were both 100.00%,the coincidence rate of bronchial artery was 98.55%,the coincidence rate of the detection of extrapulmonary systemic arteries was 72.09%,and the coincidence rate of systemic pulmonary artery shunt was 29.27%.The detection coincidence rate of the number of diseased vessels in CTA was 97.16%,compared with the results of DSA,the difference was not statistically significant (P>0.05).The pulmonary artery and bronchial artery dualphase CT could assist physicians to clearly observe the basic lung lesions of patients with hemoptysis,including pleural disease,tuberculosis,lung damage,pulmonary fibrosis,and clearly show pulmonary artery hemorrhage signs of disease and pulmonary shunt.Conclusion Dual-phase CTA imaging of pulmonary artery and bronchial artery can effectively detect the responsible blood vessel of hemoptysis in the interventional embolization treatment of hemoptysis,and provide reference for doctors to understand the position,course,opening position of diseased blood vessel,and find the pathological conditions of body artery or pulmonary artery hemorrhage,with important application value.
