Application of thromboelastogram in the evaluation of blood transfusion in patients with craniocerebral injury

Abstract   Objective To investigate the thromboelastogram (TEG) in the evaluation of blood transfusion in patients with craniocerebral injury.Methods A total of 70 patients with craniocerebral injury who were admitted to our hospital from February 2017 to June 2019 were selected,and they were divided into control group (n=35) and observation group(n=35) according to random number table method.Two groups underwent craniotomy evacuation of hematoma.The control group was supplemented with blood products based on clinical experience and blood gas analysis results during the operation,and the observation group was supplemented with blood products based on TEG test results.The intraoperative blood transfusion volume,perioperative indexes,and postoperative situations were compared between the two groups.Results The intraoperative suspended red blood cell,platelet,fresh frozen plasma and cryoprecipitation transfusion volume in the observation group were less than those in the control group (P<0.05).There was no statistically significant difference in the operation time between the two groups (P>0.05).The intraoperative bleeding volume,postoperative 6 and 24 h drainage volume in the observation group were less than those in the control group,and the differences were statistically significant (P<0.05).The rate of second craniotomy in the observation group was lower than that in the control group (P<0.05).There was no statistically significant difference in the death between the two groups within 4 weeks (P>0.05).Conclusion The application effect of TEG in patients with craniocerebral injury is significant,it can reduce intraoperative blood transfusion volume,intraoperative bleeding volume and postoperative 24 h drainage volume,and decrease the incidence of second craniotomy,which has certain guiding effect in the clinical rational use of blood products.
