基于联合评估模式的急诊老年患者谵妄管理方案应用及效果评价

Abstract Objective To explore the effects of application of delirium management program-based of joint evaluation in emergency elderly patients.Methods To establish a management plan for delirium in emergency elderly patients which based on ABCDEF joint evaluation management program by Rawson.A total of 120 emergency elderly patients admitted to Department of Emergency,Zhongshan Hospital of Traditional Chinese Medicine from January 2019 to December 2020 were divided into the observation group (admitted from January to February 2020) and the control group(admitted from January to February 2019) by the non-contemporaneous control method,with 60 cases in each group.The control group received routine delirium management program,the observation group received delirium management program-based of joint evaluation.The incidence of delirium and duration,incidence of mechanical ventilation and duration,incidence of unplanned admission to intensive care unit (ICU) and duration,the effect of nursing satisfaction were compared between the two groups.Results The incidence of postoperative delirium in the control group was lower than that of the observation group,the difference was stastically significant (P＜0.05).The duration of delirium in the observation group was shorter than that of the control group,the difference was stastically significant (P＜0.05).The duration of mechanical ventilation in the observation group was shorter than that of the control group,the difference was stastically significant (P＜0.05).The duration of unplanned admission to intensive care unit (ICU) in the observation group was shorter than that of the control group,the difference was stastically significant (P＜0.05).There was no statistically significant difference in the incidence of unplanned admission to ICU between two groups (P＞0.05).The total score of three dimensions of satisfaction with nursing services in the observation group was higher than that of the control group,the difference was stastically significant (P＜0.05).But there were no statistically significant differences in provided help when needed,skilled nursing skills,quiet room environment,and tidy entry scores between two groups(P＞0.05).Conclusion The delirium management program-based of joint evaluation in emergency elderly patients could reduced the incidence of postoperative delirium,shorter the duration of mechanical ventilation and the duration of unplanned admission to ICU,promote the satisfaction score of nursing service.
