不同剂量阿托伐他汀在糖尿病心肌病患者中的应用效果

Abstract Objective To explore the effect of different doses of Atorvastatin in patients with diabetic cardiomyopathy.Methods A total of 84 patients with diabetic cardiomyopathy admitted to the Foshan Hospital of Traditional Chinese Medicine from April 2020 to April 2021 were selected as the research objects，and they were divided into a conventional-dose group (n=42) and a high-dose group (n=42). The conventional-dose group was treated with 20 mg/d atorvastatin，and the high-dose group was treated with 40 mg/d Atorvastatin. The blood glucose indexes (fasting blood glucose [FBG]，2 h postprandial blood glucose [2 h PG]，glycosylated hemoglobin [HbA1c])，blood lipid (high density lipoprotein cholesterol [HDL-C]，total cholesterol [TC]，triglyceride [TG])，cardiac function indexes (ratio of early diastolic peak blood flow velocity to late diastolic peak blood flow velocity [E/a]，stroke volume [SV]，left ventricular ejection fraction [LVEF])，plasma brain natriuretic peptide (BNP)，serum matrix metalloproteinase-2 (MMP-2) and type Ⅲprocollagen amino terminal peptide (PⅢNP) before and after treatment，and the occurrence of adverse reactions during treatment were compared of the two groups. Results After treatment，the levels of FBG，2 h PG and HbA1c in the high-dose group were lower than those in the conventional-dose group，and the differences were statistically significant(P＜0.05). The HDL-C level of the high-dose group was higher than that of the conventional-dose group，and the levels of TC and TG were lower than those of the conventional-dose group，the differences were statistically significant (P＜0.05). The E/A，SV and LVEF of the high-dose group were higher than those of the conventional-dose group，and the differences were statistically significant (P＜0.05). The levels of BNP，MMP-2 and PIIINP in the high-dose group were lower than those in the conventional-dose group，and the differences were statistically significant(P＜0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups (P＞0.05). Conclusion Compared with conventional doses，high-dose atorvastatin can better regulate blood glucose and blood lipid levels in patients with diabetic cardiomyopathy，further improve patients' cardiac function，promote ventricular remodeling，reduce myocardial fibrosis，and have good safety.
