Value of C-reactive protein,D-dimer combined with procalcitonin in judging the severity of early acute pancreatitis |

Abstract Objective To explore the value of C-reactive protein,D-dimer combined with procalcitonin in judging the severity of early acute pancreatitis.Methods The clinical data of 69 patients with acute pancreatitis admitted to Hai′an Hospital Affiliated to Nantong University from March 2017 to March 2020 were retrospectively analyzed.The patients were divided into mild acute pancreatitis(MAP)group(n=28),moderate severe acute pancreatitis(MSAP)group(n=25)and severe acute pancreatitis(SAP)group (n=16)according to 2019 Chinese guidelines for diagnosis and treatment of acute pancreatitis.The levels of C-reactive protein,D-dimer and procalcitonin in the three groups were analyzed,and the diagnostic value of the combined detection of the three groups of indicators was analyzed.Results C-reactive protein,D-dimer and procalcitonin in SAP group and MSAP group were higher than those in MAP group,while C-reactive protein,D-dimer and procalcitonin in SAP group were higher than those in MSAP group,the differences were statistically significant (P<0.05).The sensitivity,specificity and accuracy of the combined detection of three indexes for severe acute pancreatitis were 93.75%,100% and 98.55%.Multivariate logistic analysis showed that D-dimer (β=2.330,OR=4.575,95%CI=1.354-7.957),C-reactive protein (β=1.125,OR=4.182,95%CI=1.538-6.175)and procalcitonin (β=2.232,OR=3.869,95%CI=1.287-7.203)were independent influencing factors of SAP (P<0.05).Conclusion Compared with the single index or the combination of two indexes,the combined detection of C-reactive protein+D-dimer+procalcitonin is more accurate in judging the severity of acute pancreatitis,which is worthy of clinical promotion.
