Abstract Objective To compare the clinical effect of non-surgical treatment and surgical treatment of uremic hip femoral neck fracture.Methods The clinical data of 60 hospitalized patients with uremic hip femoral neck fracture in the Department of Orthopedics and Traumatology of Jiujiang Hospital of Traditional Chinese Medicine from June 2018 to December 2020 were analyzed retrospectively,according to different treatment methods,they were divided into the non-surgical treatment group(30 cases)and the surgical treatment group(30 cases).The non-surgical treatment group was treated with skin traction and bone traction,and the surgical treatment group was treated with cannulated screw internal fixation.The degree of pain and swelling of the two groups before treatment and 1,2 and 4 weeks of treatment were recorded.At the same time,the hip function scores of the two groups at 8 and 12 weeks after treatment were recorded,and the complications of the two groups were observed.Results There were no significant difference in swelling and pain scores between the two groups before treatment and 1 week after treatment(P>0.05).The swelling and pain scores of the surgical treatment group at 2 and 4 weeks after treatment were lower than those of the non-surgical treatment group,the differences were sta-tistically significant(P<0.05).There were no significant difference in pain scores of the non-surgical treatment group between 1,2 week after treatment and that before treatment(P>0.05).There were no significant difference in pain and swelling scores of the non-surgical treatment group between 2 week after treatment and 1 week after treatment(P>0.05).The pain score of the non-surgical treatment group after 4 weeks of treatment was lower than that before treatment,the difference was statistically significant(P<0.05).The pain and swelling scores of the non-surgical treatment group at 4 weeks after treatment were lower than those at 1 and 2 weeks after treatment,the differences were statistically significant(P<0.05).The swelling scores of the non-surgical treatment group at 1,2 and 4 weeks after treatment were lower than those before treatment,the differences were statistically significant(P<0.05).There was no significant difference in pain score of the surgical treatment group between 1 week after treatment and that before treatment(P>0.05).The swelling score of the surgical treatment group at 1 week after treatment was lower than that before treatment,the difference was statistically significant(P<0.05).The swelling and pain scores in the surgical treatment group at 2 and 4 weeks after treatment were lower than those before treatment,the differences were statistically significant(P<0.05).The scores of pain and swelling in the surgical treatment group after 2 weeks of treatment were lower than those after 1 week of treatment,the differences were statistically significant(P<0.05).The swelling and pain scores of the surgical treatment group at 4 weeks after treatment were lower than those at 1 and 2 weeks after treatment,the differences were statistically significant(P<0.05).The hip function score of the two groups at 12 weeks of treatment were higher than those at 8 weeks of treatment,the differences were statistically significant(P<0.05).The hip function score of the surgical treatment group was higher than that of the non-surgical treatment group at 8 and 12 weeks,the differences were statistically significant(P<0.05).The hip function score of the surgical treatment group at 8 and 12 weeks of treatment were higher than those of the non-surgical treatment group,the differences were statistically significant(P<0.05).12 weeks after treatment,the total incidence of complications in the surgical treatment group was 10.00%,which was lower than 33.33% in the non-surgical treatment group,the difference was statistically significant(P<0.05).Conclusion For patients with uremic hip femoral neck fracture,surgical treatment can quickly reduce the pain and swelling of patients,promote the recovery of hip function,and the total incidence of complications is low.It can be used as the first choice of treatment.
