Abstract Objective To explore the application value of early atomization inhalation of Budesonide under ventilator in the prevention of bronchopulmonary dysplasia (BPD)in premature infants.Methods The clinical data of 60 premature infants who needed ventilator treatment in Jingdezhen Maternal and Child Health Hospital from January 2020 to January 2023 were retrospectively analyzed.According to the different treatment methods,they were divided into control group (30 cases)and observation group(30 cases).The control group was treated with noninvasive ventilator assisted ventilation,and the observation group was treated with noninvasive ventilator assisted ventilation and early atomization inhalation of Budesonide under ventilator.The occurrence of BPD,treatment effect,pulmonary oxygenation index,inflammatory factors and adverse reactions were compared between the two groups.Results The incidence of BPD in the treatment group was lower than that in the control group,and the total oxygen inhalation time and ventilator use time were shorter than those in the control group,and the differences were statistically significant(P<0.05).The oxygenation index(OI),alveolar-arterial oxygen partial pressure difference(PA-aO2),arterial partial pressure of carbon dioxide (PaCO2),tumor necrosis factor-α (TNF-α),interleukin(IL) -6,IL-8 and procalcitonin (PCT) in the treatment group were lower than those in the control group after 48 hours of treatment.The arterial partial pressure of oxygen (PaO2)in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).There were no adverse reactions in the two groups during treatment.Conclusion The application of early aerosol inhalation of budesonide under ventilator in premature infants can better improve their lung oxygenation indicators,shorten the total oxygen inhalation time and ventilator use time,reduce the levels of TNF-α,IL-6 and other inflammatory factors,and actively prevent the occurrence of BPD.It is safe and reliable,and has high clinical application value.
