Clinical application of endoscopic band ligation in colon diverticulum hemorrhage |

Abstract Objective To explore the therapeutic effect of endoscopic band ligation (EBL) on colon diverticulum hemorrhage.Methods A total of 72 patients with colon diverticulum hemorrhage admitted to the People's Hospital of Ganzhou from January to December 2021 were selected as the study objects.The patients were divided into EBL group(36 cases)and control group (36 cases) by random number table method.The control group received endoscopic metal clip hemostasis treatment,while the EBL group received EBL treatment.The operative index(operation time,intraoperative hemostatic time,length of hospital stay),blood coagulation indicators (activated partial thromboplastin time [APTT],prothrombin time[PT],plasma fibrinogen [FIB])before and 24 h after operation were compared between the two groups.Stress hormones(cortisol[Cor],norepinephrine [NE],adrenocorticotropin [ACTH]),acute phase proteins (C-reactive protein [CRP],α1-antitrypsin[α1-AT],serum amyloid A [SAA]),postoperative adverse events were recorded,the success rate of initial hemostasis,and the rate of late bleeding were followed up 1 year after surgery.Results The operation time,hemostasis time and hospitalization time in EBL group were shorter than those in the control group,with statistical significances(P<0.05).APTT and PT in EBL group were shorter than those in control group,and FIB level was higher than that in control group,with statistical significances (P<0.05).The levels of Cor,NE and ACTH in EBL group were lower than those in control group,with statistical significances (P<0.05).The levels of CRP,α-1-AT and SAA in EBL group were lower than those in control group,with statistical significances (P<0.05).There was no statistical significant difference in the incidence of adverse events between the two groups(P>0.05).The successful rate of first hemostasis in EBL group was higher than that in the control group,and the late bleeding rate was lower than that in the control group,with statistical significances(P<0.05).Conclusion EBL can effectively shorten the hemostasis time,operation time and hospital stay,improve the blood coagulation function and the expression of stress hormone,and has a good therapeutic effect on colon diverticulum hemorrhage.
