Analysis of the composition and homology of non-single urinary stones in vivo by CT energy spectrum

Abstract  Objective To use CT energy spectrum to explore the in vivo stone composition of patients with non-single urinary stones,and to conduct homology analysis.Methods A total of 102 cases were collected from June 2018 to December 2019 who went to Foshan Sanshui District People′s Hospital for urinary system CT energy spectrum analysis and scanning of stones,and all cases were detected by infrared spectroscopy.Divide the same patient′s urinary tract stones into 7 types according to the stone location:ipsilateral kidney,opposite kidney,ipsilateral kidney ureter,opposite kidney ureter,opposite ureter,bladder,and bladder+kidney/ureter.Energy spectrum stone analysis software was used to measure average effective atomic number,peak value of effective atomic number,50 keV single energy CT value,calcium to water ratio,slope of energy spectrum attenuation curve,140 kVp mixed energy CT value,and determine the stone composition;the same patient in the same body stone composition was judged as homology,otherwise it was heterogeneity.Results A total of 204 stones were analyzed by CT energy spectrum of 102 patients,and the coincidence rate of in vivo energy spectrum and infrared spectrometer was 85.3%.The results of determining stone composition were calcium phosphate (51 stones),calcium oxalate(85 stones),struvite(25 stones),cystine(6 stones),uric acid(23 stones)and mixed(14 stones).Comparison of the average effective atomic number,peak value of effective atomic number,50 keV single energy CT value,calcium to water ratio,slope of energy spectrum attenuation curve,and 140 kVp mixed energy CT value of the five single-component stones,the differences were statistically significant(P<0.01).Patients with homology accounted for 75.5%,mainly bladder and ipsilateral kidney ureter.The 7 types of homology from large to small are bladder,ipsilateral kidney ureter,opposite kidney,ipsilateral kidney,opposite ureter,opposite kidney ureter,bladder+kidney/ureter.Conclusion CT energy spectrum imaging can more accurately analyze the components of urinary stones in vivo.The components of non-single urinary stones in the same patient have high homology,especially the bladder and ipsilateral kidney ureter.
