不同激素量联合环磷酰胺及雷公藤治疗中老年肾病综合征的效果

摘要目的探讨不同激素量联合环磷酰胺及雷公藤治疗中老年肾病综合征的疗效。方法选取2014年3月～2016年2月我院收治的中老年肾病综合征患者100例，按照抽签法分成Ⅰ、Ⅱ、Ⅲ、Ⅳ四组，每组25例。在给予基础性用药（环磷酰胺联合雷公藤）的前提下，分别使用不同剂量的醋酸泼尼松进行治疗。四组患者的醋酸泼尼松用药剂量和用药方法如下：Ⅰ组（0.4 mg/（kg·d）；Ⅱ组（0.6 mg/（kg·d）；Ⅲ组（0.8 mg/（kg·d）；Ⅳ组（1.0 mg/（kg·d），均为晨起顿服。对比治疗3个月后，四组患者的治疗效果。结果在接受为期3个月时间的连续性治疗后，Ⅳ组患者的临床治疗有效率为100%，明显高于其他三组的92%，且在尿蛋白定量、血浆清蛋白水平、以及血肌酐等指标的改善情况，也明显优于其他三组，差异有统计学意义（P＜0.05）。结论环磷酰胺+雷公藤+醋酸泼尼松治疗中老年肾病综合征可取得较好的效果，尤其是当激素类药物—醋酸泼尼松的使用剂量为1.0 mg/（kg·d）时，治疗效果最佳，因此，具有较好的临床使用和推广价值。

Abstract：Objective To explore the clinical effect of different dosage hormone combined with Cyclophosphamide and Tripterygium Wilfordii in the treatment of nephrotic syndrome in middle and old aged patients.Methods 100 middle and old aged patients with nephrotic syndrome in our hospital from March 2014 to February 2016 were selected.Patientsweredividedinto Ⅰ,Ⅱ,Ⅲ and Ⅳgroups,andeach group was 25 cases.On the basis of Cyclophosphamide and Tripterygium Wilfordii,patients in different groups were treated with different doses of prednisone acetate.The dosage and medication of prednisone acetate for four groups of patients were as follows： group Ⅰ[0.4 mg/(kg·d)];group Ⅱ[0.6 mg/(kg·d)];group Ⅲ[0.8 mg/(kg·d)];group Ⅳ[1.0 mg/(kg·d)],and were taken in every morning.3 months later,the treatment effect among four groups were compared.Results After 3 months of continuous treatment,the effective rate of clinical treatment in groupⅣwas 100%,significantly higher than 92%in the other three groups,and in the improvement of urine protein content,plasma albumin level,blood creatinine and other indicators,it was also significantly better than the other three groups,and the difference was significant(P＜0.05).Conclusion Cyclophosphamide combined with Tripterygium Wilfordii and prednisone acetate in the treatment of nephrotic syndrome in middle-aged and elderly patients can achieve better results,especially when the dose hormone drug prednisone acetate is 1.0 mg/(kg·d),the treatment is best.Therefore,it has good clinical application and promotion value.
