Application effect of Shengxuebao Mixture and iron supplement in patients with anemia after unilateral or bilateral total hip arthroplasty

摘要目的 探讨生血宝合剂联合补铁剂在单侧或双侧人工全髋关节置换术后贫血患者中的应用效果。方法 选取2018年1月~2019年5月我院收治的124 例接受单侧或双侧人工全髋关节置换术且术后出现贫血患者作为研究对象,随机分为对照组和观察组,每组各62 例。对照组术后接受蔗糖铁溶液静脉输注,观察组术后接受蔗糖铁溶液+生血宝合剂联合用药。比较两组手术前后的血液指标、治疗效果、中医症候评分治疗效果、髋关节功能(Harris)评分及药物不良反应发生情况。结果 两组术后1、7、14 d 的红细胞计数(RBC)低于术前,差异均有统计学意义(P<0.05);观察组术后1、7 d 的血红蛋白(Hb)水平低于术前,对照组术后1、7、14 d 的Hb 水平低于术前,差异均有统计学意义(P<0.05);观察组术后1 d 的红细胞压积(HCT)低于术前,对照组术后1、7 d 的HCT 低于术前,差异均有统计学意义(P<0.05)。观察组术后7、14 d 的RBC、Hb 和HCT 高于对照组,差异均有统计学意义(P<0.05)。观察组的治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组的中医症候评分总有效率高于对照组,差异有统计学意义(P<0.05)。观察组术后7、14 d 的Harris 评分高于对照组,差异均有统计学意义(P<0.05)。两组的不良反应发生率比较,差异无统计学意义(P>0.05)。结论 生血宝合剂与补铁剂联合用于全髋关节置换术后贫血能改善患者的血液学指标、中医症候和髋关节功能,提高患者的治疗效果,不增加不良反应的发生,值得临床推广应用。
Abstract:Objective To investigate the application effect of Shengxuebao Mixture combined with iron supplement in patients with anemia after unilateral or bilateral total hip arthroplasty. Methods A total of 124 patients who underwent unilateral or bilateral total hip arthroplasty and had anemia after surgery in our hospital from January 2018 to May 2019 were selected as the research objects, they were randomly divided into the control group and observation group,62 cases in each group. The control group received an intravenous infusion of Iron Sucrose Solution, the observation group received a combination of Iron Sucrose Solution and Shengxuebao Mixture. The blood index before and after operation, treatment effect, traditional Chinese medicine symptom score treatment effect, hip joint function (Harris) score,and adverse drug reactions were compared between the two groups. Results The RBC of the two groups at 1,7,14 days after operation were lower than those before operation, the differences were statistically significant (P <0.05). The hemoglobin (HB) level of the observation group at 1, 7 days after operation were lower than those of before operation,The Hb level of the control group at 1,7,14 days after operation were lower than those of before operation, the differences were statistically significant (P<0.05). The hematocrit (HCT) of the observation group at 1 day after operation was lower than that of before operation, the HCT of the control group at 1, 7 days after operation were lower than those of before operation, the differences were statistically significant (P<0.05). The RBC, Hb and HCT of the observation group were higher than those of the control group at 7, 14 days after operation, the differences were statistically significant(P<0.05). The total effective rate of the observation group was higher than that of the control group, the difference was statistically significant (P<0.05). The total effective rate of traditional chinese medicine symptom score in the observation group was higher than that in the control group, the difference was statistically significant (P<0.05). The Har-ris score of the observation group at 7 and 14 days after operation were higher than those of the control group, the differences were statistically significant (P<0.05). There was no significant difference of the incidence of adverse reactions between the two groups (P>0.05). Conclusion The combination of Shengxuebao Mixture and iron supplement for anemia after total hip arthroplasty can improve patients′ hematological indexes, traditional chinese medicine symptoms, and hip function, improve the treatment effect of patients, and do not increase the incidence of adverse reactions. It is worthy of clinical application.
