拖线置管术和切开挂线术治疗高位复杂性肛瘘的效果比较及对肛管直肠压力的影响

摘要目的比较拖线置管术和切开挂线术治疗高位复杂性肛瘘的效果及对肛管直肠压力的影响。方法选择2013年9月～2015年6月前来我院就诊的高位复杂性肛瘘患者84例，将其随机分为两组，实验组42例采用拖线置管术进行治疗，对照组42例采用切开挂线术进行治疗。比较两组的治愈率与愈合时间，距肛门20、30mm处的肛管直肠压力（20、30mm收缩压与20、30 mm静息压），随访6个月的复发率。结果实验组愈合时间明显短于对照组（P＜0.05）。与术前比较，两组愈合后3个月的20、30mm收缩压与20、30mm静息压均降低（P＜0.05）；实验组愈合后3个月的20、30mm收缩压与20、30mm静息压与对照组比较均升高，差异有统计学意义（P＜0.05）。两组的治愈率、随访6个月的复发率差异无统计学意义（P＞0.05）。结论拖线置管术治疗高位复杂性肛瘘的效果优于切开挂线术，治愈时间缩短，对肛管直肠压力影响小，值得推广。

Abstract：ObjectiveTo compare the effect of drag line intubation and incision-thread-drawing procedure treating high complex anal fistula and influence on anorectal pressure.Methods84 patientswith high complex anal fistula treated by our hospital from September 2013 to June 2015 were selected and they were randomly divided into two groups and experimental group(n=42)was treated by drag line intubation,control group(n=42)was treated by incision-thread-drawing procedure.Cure rate,healing time,anorectal pressure from the anus20mm and 30mm(20mm and 30mm systolic pressure and resting pressure)and recurrence rate when patients were followed-up for 6 months between two groups was compared respectively.ResultsThe healing time of experimental group was significantly shorter than that of control group(P＜0.05).Compared with before operation,20mm and 30mm systolic pressure and resting pressure after 3months healing in two groupswere decreased(P＜0.05).20mm and 30mm systolic pressure and resting pressure after 3months healing in experimental group were higher than those of control group (P＜0.05).There was no significant difference of cure rate and recurrence rate when patientswere followed-up for 6months(P＞0.05).ConclusionThe effect of drag line intubation treating high complex anal fistula is better than that of incision-thread-drawing procedure,cure time is shortened,and it has little influence on anorectal pressure.It isworthy of promotion.
