Abstract：Objective To explore the value of transperineal pelvic floor ultrasound in the diagnosis of outlet obstructive constipation.Methods A total of 80 patients with constipation who visited the Zhongshan City People's Hospital in Guangdong Province from March 2022 to December 2022 were selected as the study subjects,all subjects underwent transperineal pelvic floor ultrasound diagnosis and X-ray defecography examination.The results of X-ray defecography were taken as the gold standard,the accuracy,sensitivity,specificity,positive predictive value and negative predictive value of transperineal pelvic floor ultrasound diagnosis were calculated.The accuracy of diagnosing for different conditions and patient satisfaction using of two diagnostic methods were compared.Results X-ray defecography showed positive results in 69 cases and negative results in 11 cases.Ultrasound diagnosis of the transperineal pelvic floor showed positive results in 71 cases and negative results in 9 cases.The results of X-ray defecography were taken as the gold standard,the consistency of transperineal pelvic floor ultrasound diagnosis was shown to be Kappa=0.951, P ＜0.001,indicated a high degree of consistency.The results of X-ray defecography were taken as the gold standard,the sensitivity of transperineal pelvic floor ultrasound diagnosis was 94.20% (65/69),specificity was 45.45% (5/11),negative predictive value was 55.56% (5/9),positive predictive value was 91.55% (65/71) and accuracy was 87.50% (70/80).There was no statistically significant difference in the diagnostic accuracy of two inspection methods for different conditions (P＞0.05).The satisfaction level of patients with X-ray defecography diagnosis was lower than that with transperineal pelvic floor ultrasound diagnosis,the difference was statistically significant (P ＜0.05).Conclusion In the diagnosis of outlet obstructive constipation,the diagnostic results of X-ray defecography are used as the gold standard.The sensitivity and consistency of the results of transperineal pelvic floor ultrasound diagnosis are high,the pelvic tissue can be clearly observed,patients' satisfaction is high,in clinical diagnosis,appropriate methods or joint diagnosis should be selected based on the patient's condition to improve diagnostic efficiency.
