Study on the occurrence and risk factors of ventricular premature beats in the third trimester of pregnancy and its influence on the maternal and infant outcome

Abstract:Objective To investigate the occurrence and risk factors of ventricular premature beats (VPB) in the third trimester of pregnancy and its influence on the maternal and infant outcomes.Methods Eighty pregnant women who underwent pregnancy examination in Xinyu Maternal and Child Health Hospital from October 2019 to October 2022 were selected as the study subjects.All of them underwent 12 lead ECG examination.They were divided into normal pregnancy group and combined with VPB group according to whether they were accompanied with VPB.Basic data such as age,body weight,delivery status,education level,hyperlipidemia,gestational hypertension,gestational diabetes,combined with primary heart disease,anxiety and depression were collected in the two groups.Univariate analysis was carried out first,and logistic regression analysis was performed for items with statistical differences to obtain independent risk factors affecting the occurrence of VPB in the third trimester.The outcomes of the two groups were compared.Results A total of 28 cases of premature ventricular beat were detected in 80 pregnant women in the third trimester,the incidence was 35.00% (28/80).The results of univariate analysis showed that there were statistically significant differences in age,body weight,hyperlipidemia,gestational hypertension,gestational diabetes,combined primary heart disease,anxiety and depression between the two groups(P<0.05).Multivariate logistic analysis showed that age≥35 years old(β=2.920,OR=18.545,95%CI:5.203-66.107),body weight≥75 kg (β=1.310, OR=3.706,95%CI: 1.410-9.738),hyperlipidemia (β=1.870, OR=6.491,95%CI:2.329-18.092),gestational hypertension (β=1.852,OR=6.370,95%CI:2.257-17.978),gestational diabetes mellitus(β=1.848,OR=6.346,95%CI: 2.203-18.278),combined with primary heart disease (β=2.197,OR=9.000,95%CI:2.540-31.893),anxiety (β=1.746,OR=5.730,95%CI: 2.103-15.611),depression (β=1.819,OR=6.167,95%CI:2.233-17.032)were the high risk factors for the occurrence of VPB in the third trimester of pregnancy (P<0.05).The total incidence of adverse maternal and infant outcomes in the group with VPB was higher than that in the normal pregnancy group,and the difference was statistically significant (P<0.05).Conclusion The incidence of VPB in the third trimester of pregnancy is relatively high,which is closely related to age≥35 years,body mass ≥75 kg,hyperlipidemia,pregnancy induced hypertension,pregnancy induced diabetes,primary heart disease,anxiety,depression.VPB can increase the risk of adverse maternal and infant outcomes,and targeted prevention and treatment should be carried out as soon as possible.
