Abstract:Objective To investigate the value of ultrasonography detection of fetal middle cerebral artery and umbilical artery blood flow parameters and fetal distress in utero.Methods A total of 160 pregnant women who gave birth in Shenzhen Hospital of the University of Hong Kong from October 2019 to December 2021 were selected as the research objects.According to whether intrauterine distress occurred,the patients were divided into distress group (with intrauterine distress,80 cases)and control group (without intrauterine distress,80 cases).Both groups were examined by ultrasound.Blood flow parameters of middle cerebral artery and umbilical artery were compared between the two groups.According to the Apgar score,the degree of neonatal hypoxia was divided into mild and severe,and the blood flow parameters of middle cerebral artery and umbilical artery were stratified and compared.The correlation between the blood flow parameters of middle cerebral artery and umbilical artery and Apgar score was analyzed by linear correlation analysis.Results The middle cerebral artery resistance index (RI),pulsatility index (PI)and peak systolic/diastolic blood flow velocity (S/D)values of fetuses in the distress group were lower than those in the control group,and the differences were statistically significant(P<0.05).The umbilical artery RI,PI and S/D values of the fetuses in the distress group were higher than those in the control group,and the differences were statistically significant(P<0.05).The area under the curve(AUC)of RI,PI and S/D values in the diagnosis of fetal distress were 0.779,0.863 and 0.778,respectively.The AUC of umbilical artery RI,PI and S/D in the diagnosis of fetal distress were 0.794,0.844 and 0.750,respectively.The RI,PI and S/D values of middle cerebral artery in severe hypoxia fetuses were higher than those in mild hypoxia fetuses,and the differences were statistically significant(P<0.05).The umbilical artery RI,PI and S/D values of severe hypoxia fetuses were lower than those of mild hypoxia fetuses,and the differences were statistically significant (P<0.05).In the distress group,RI,PI,S/D values of the middle cerebral artery were positively correlated with Apgar score at 1 min after birth (P<0.05).The umbilical artery RI,PI,S/D values of fetal distress group were negatively correlated with neonatal Apgar score at 1 min after birth(P<0.05).Conclusion Ultrasound detection of fetal middle cerebral artery and umbilical artery blood flow parameters can diagnose the occurrence of intrauterine distress and is related to the degree of neonatal hypoxia.
