Clinical significances of detection of platelet counting,coagulation indexes and thromboelastogram in patients with hypertensive disorder complicating pregnancy

Objective To investigate the clinical significances of detection of platelet counting (PLT),coagulation indexes and thromboelastogram(TEG) in patients with hypertensive disorder complicating pregnancy (HDP).Methods The clinical datum of 44 patients with gestational hypertension(the GH group),42 patients with preeclampsia(the PE group),63 patients with severe preeclampsia(the SPE group)and 155 healthy pregnant women(the control group)admitted into Jiangxi Maternal and Child Health Hospital from January 2019 to March 2021 were analyzed retrospectively.The baseline datum of all subjects were collected,as well as PLT,prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(Fg),coagulation reaction time(R value),coagulation formation time(K value),coagulation angle(α Angle)and maximum thrombus amplitude (MA)during late pregnancy and so on.Results There were significant differences in the levels of PLT,APTT,TT and Fg during late pregnancy among the four groups(P<0.05),but there was no significant difference in the level of PT (P>0.05).Further pairwise comparisons showed that the level of PLT during late pregnancy in the PE group were higher than those in the GH group and the SPE group,the differences were statistically significant (P<0.05).The levels of APTT and TT during late pregnancy in the PE group and the SPE group were lower than those in the GH group and the control group,the difference were statistically significant (P<0.05).The levels of Fg during late pregnancy in the GH group,the PE group and the SPE group were higher than those in the control group,the differences were statistically significant (P<0.05).The levels of Fg during late pregnancy in the GH group were lower than those in the PE group and the SPE group,the differences were statistically significant (P<0.05),and there were no significant differences in other indexes between any two groups(P>0.05).There were significant differences in the R value,K value,α Angle and MA value during late pregnancy among the four groups (P<0.05).Further pairwise comparisons showed that the R value and K value during late pregnancy in the SPE group were lower than those in the other groups,the differences were statistically significant (P<0.05),while the α Angle and MA value were higher than those in other groups,the differences were statistically significant (P<0.05).The R value and K values during late pregnancy in the PE group were lower than those in the GH group and the control group,the differences were statistically significant (P<0.05),while the α Angle and MA value were higher than those in the GH group and the control group,the differences were statistically significant (P<0.05),and there were no significant differences in the R value,K value,α Angle and MA value during late pregnancy between the GH group and the control group(P>0.05).Conclusion There is an obvious hypercoagulable states in the blood of HDP patients during late pregnancy,and the blood hypercoagulable states becomes more and more serious with the aggravation of the disease,then the coagulation status of HDP patients can be monitored from many aspects.
