Abstract:Objective To understand the situation of medication errors reported in Gejiu People's Hospital,and provide analysis and preventive measures for the hospital.Methods The medication error reports reported in Gejiu People's Hospital from January 2014 to December 2020 were collected.The classification,error content,triggering factors,cases and composition of triggering and discovering personnel were statistically analyzed.Results Among the 253 medication errors reported,the proportion of men and women was equal (52.17% for men and 47.83% for women).Patients over 60 years old caused the most medication error reports,accounting for 152 cases (60.08%).The top three reported quantities of medication errors were dosage of 33 cases (13.04%),interaction of 28 cases (11.07%) and contraindications of 26 cases (10.28%).More than 150 kinds of drugs were involved,and the dosage form was mainly injection.Conclusion The classification of medication errors in Gejiu People's Hospital is mainly Grade B,and the content of errors is mainly dosage errors.The main personnel and factors causing medication errors are the lack of professional knowledge and training of residents.Pharmacists play an important role in finding and correcting errors in time.
