Abstract:Objective To investigate and analyze the application of Esketamine combined with femoral nerve block laryngeal general anesthesia in knee arthroscopic surgery.Methods A total of 72 patients with knee arthroscopy in the First Hospital of Nanping City from March 2020 to May 2021 were selected as the research subjects.The patients were divided into control group and observation group by random number table method,36 cases in each group.The control group adopted ultrasound-guided femoral nerve block after the onset of laryngeal general anesthesia,and the observation group adopted Esketamine on the basis of the control group.The heart rate (HR) and mean arterial pressure (MAP)of the two groups were compared before anesthesia (T0),before skin incision (T1),immediately after skin incision (T2),intraoperative exploration (T3) and after surgery (T4).The duration of sensory block and motor block were compared between the two groups.Visual analogue scale (VAS) scores were compared between the two groups immediately after extubation and 0.5,8,and 24 h after operation.Perioperative adverse reactions were compared between the two groups.Results There were no significant differences in HR and MAP at T0 between the two groups (P>0.05).HR of T1,T2,T3 and T4 in the observation group were higher than those in the control group,and MAP of T1,T2 and T3 were higher than those in the control group,and the differences were statistically significant (P<0.05).In the control group,the HR of T1,T2,T3 and T4 was lower than that of T0,and the MAP of T1,T2 and T3 was lower than that of T0.In the observation group,MAP of T3 was lower than that of T0,and the differences were statistically significant (P<0.05).There was no significant difference in the duration of sensory block and motor block between the two groups (P>0.05).The VAS scores of the observation group immediately after extubation,0.5 and 8 h after operation were lower than those of the control group,and the differences were statistically significant (P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Ultrasound-guided femoral nerve block and laryngeal mask general anesthesia with the addition of Esketamine in knee arthroscopy to ensure more stable hemodynamics,and the postoperative pain level is low,doesn't increase perioperative adverse reactions,and has higher safety.
