Value of red blood cell distribution width in prediction of fatality rate of patients with ST-segment elevation myocardial infarction after percutaneous coronary intervention

Abstract: Objective To investigate the value of red blood cell distribution width (RDW) in prediction of fatality rate of patients with ST-segment elevation myocardial infarction (STEMI) after percutaneous coronary intervention (PCI).Methods The clinical data of 80 STEMI patients who were treated with PCI in Jingdezhen Second People′s Hospital from February 2017 to February 2019 were retrospectively analyzed.The clinical data of 18 dead patients during hospitalization were classified in observation group,and 62 survived patients during hospitalization were classified in control group.The levels of RDW,white blood cell count (WBC),cardiac troponin I (cTn-Ⅰ) were detected.Binary Logistic regression analysis was used to examine the effect of RDW level on the fatality rate of STEMI patients after PCI,and the area under the ROC curve was used to evaluate the predictive value of RDW and cTnⅠin fatality rate of STEMI patients after PCI.Results There was no statistical difference in the WBC level between two groups (P>0.05).The levels of RDW and cTn-Ⅰin observation group were higher than those in control group,and the differences were statistically significant (P<0.05).Binary Logistic regression analysis showed that the RDW and cTn-Ⅰlevels were the risk factors for the fatality rate of STEMI patients after PCI (OR>1,P<0.05).ROC curve showed that the AUC of RDW was 0.711 (95%CI:0.572-0.851),which was ≥0.7,with better predictive value.When the optimal cut-off value was 11.180,the ideal sensitivity and specificity can be obtained,which were 96.60%,85.50%,respectively.The AUC of cTn-Ⅰwas 0.657 (95%CI:0.520-0.794),0.5≤AUC<0.7,with lower predictive value,when the optimal cut-off value was 2.555,the ideal sensitivity and specificity can be obtained,which were 94.40%,58.10%,respectively.Conclusion Elevated RDW and cTn-Ⅰlevels are the risk factor for mortality in patients with STEMI after PCI.The higher the RDW and cTn-Ⅰlevels,the greater the risk of fatality rate in STEMI patients after PCI.Clinically,the patients with this condition should be given high attention.
