Objective To investigate the preventive effect of Caffeine on bronchopulmonary dysplasia (BPD) in premature infants and its influence on the level of vascular endothelial growth factor (VEGF). Methods A total of 42 premature infants treated in the Department of Neonatal Pediatrics of the First Affiliated Hospital of Shaoyang University from May 2020 to April 2021 were selected as the research objects, according to random number table method, they were divided into the treatment group and the control group, with 21 cases in each group. The control group was treated with routine symptomatic treatment, and the treatment group was treated with Caffeine on the basis of the control group. The hospitalization time, weight growth rate, aerobic time, incidence rate of BPD, mortality rate, and the expression level of VEGF in peripheral blood serum on 1, 3, 7, 14, 21 d after birth of the two groups were compared. Results The hospitalization time and aerobic time of the premature infants in the treatment group were shorter than those in the control group, the weight growth rate in the treatment group was faster than that in the control group, he incidence rate of BPD in the treatment group was lower than that in the control group, the differences were statistically significant (P<0.05).There was no significant difference in mortality between the treatment group and the control group (P>0.05). At the three time points of 7, 14, 21 d, the expression level of VEGF in the serum of premature infants in the treatment group were lower than those in the control group, the differences were statistically significant (P<0.05). Conclusion The expression level of VEGF is related to the time of Caffeine treatment of BPD, which has a certain clinical significance
