改良Brisson术在隐匿型阴茎患儿中的应用效果

摘要目的探讨改良Brisson 术在隐匿型阴茎患儿中的应用效果。方法选取2020年3月至2021年4月普宁市人民医院收治的120 例隐匿型阴茎患儿作为研究对象，按随机数字表法分为对照组和观察组，每组各60 例。对照组采用常规内外板单瓣嵌插术，观察组采用改良Brisson 术，两组均于术后随访1 个月。比较两组手术效果、术前及术后1 个月生活质量、术后7 d 并发症、术后1 个月满意度。结果两组的手术效果比较，观察组的水肿恢复时间、绷带包扎时间均短于对照组，术后1 个月阴茎增长长度长于对照组、阴茎外观评分高于对照组，差异有统计学意义（P＜0.05）。两组术前生活质量比较，差异无统计学意义（P＞0.05）；两组术后1 个月的生活质量比较，自我满意度、躯体感受、生活便利性评分均高于同组术前，且观察组均高于对照组，差异有统计学意义（P＜0.05）。观察组术后7 d 的并发症总发生率高于对照组，差异有统计学意义（P＜0.05）。观察组术后1 个月的总满意度高于对照组，差异有统计学意义（P＜0.05）。结论改良Brisson 术能有效促进隐匿型阴茎患儿阴茎增长，减轻包皮水肿程度，降低术后并发症的发生率，提高生活质量及整体满意度。

Abstract：Objective To investigate the effect of modified Brisson operation in children with concealed penis. Methods A total of 120 children with concealed penis treated in Puning People′s Hospital from March 2020 to April 2021 were selected as the research object. Children with concealed penis were randomly divided into control group and observation group, with 60 cases in each group. The control group was treated with conventional single valve insertion of inner and outer plates, and the observation group was treated with modified Brisson operation. Both groups were followed up for 1 month. The operation effect, quality of life before and 1 month after operation, complications 7 days after operation, and satisfaction 1 month after operation were compared between the two groups. Results Compared with the operation effect of the two groups, the edema recovery time and bandage dressing time in the observation group were shorter than those in the control group, the penis growth length was longer than that in the control group one month after operation, and the penis appearance score was higher than that in the control group, with statistically significant differences (P＜0.05). There was no significant difference in preoperative quality of life between the two groups (P＞0.05).Compared with the quality of life between the two groups one month after operation, the scores of self satisfaction, physical feeling and life convenience in the two groups were higher than those before operation, and the scores in the observation group were higher than those in the control group,with statistically significant differences(P＜0.05).The total incidence of complications 7 days after operation in the observation group was higher than that in the control group, and the difference was statistically significant (P＜0.05). The total satisfaction of the observation group one month after operation was higher than that of the control group,with a statistically significant difference(P＜0.05).Conclusion The modified Brisson operation can effectively promote the growth of penis in children with concealed penis, reduce the degree of prepuce edema, reduce the incidence of postoperative complications, and improve the quality of life and overall satisfaction.
