基于循证理念的手术室感染护理对老年髋关节置换术后患者感染发生情况的影响

摘要目的探究在老年髋关节置换术后患者中实施基于循证理念的手术室感染护理对感染发生情况的影响。方法回顾性分析2018年7月至2021年8月于厦门大学附属中山医院行髋关节置换术的108 例患者临床资料，2018年7月至2020年1月实施常规护理期间收治54 例患者设为对照组，2020年2月至2021年8月实施基于循证理念的手术室感染护理期间收治的54 例患者设为观察组。比较两组患者的住院时间、手术室感染检测合格率、感染总发生率及护理总满意度。结果观察组住院时间为（14.82±2.53）d，短于对照组的（19.24±3.72）d，差异有统计学意义（P＜0.05）；两组的手术室空气、无菌包合格率比较，差异无统计学意义（P＞0.05）；观察组的医护人员手、消毒液、物体表面检测合格率分别为98.15%、94.44%、92.59%，均高于对照组的83.33%、77.78%、75.93%，差异有统计学意义（P＜0.05）；观察组的感染总发生率为7.41%，低于对照组的22.22%，差异有统计学意义（P＜0.05）；观察组的护理总满意度为98.15%，高于对照组的83.33%，差异有统计学意义（P＜0.05）。结论基于循证理念的手术室感染护理可有效提高手术室感染检测合格率，降低感染率，缩短住院时间，获得较高护理满意度。

Abstract：Objective To explore the effect of implementing evidence-based concept of operating room infection care on infection in elderly patients after hip replacement.Methods The clinical data of 108 patients who underwent hip arthroplasty in Zhongshan Hospital Affiliated to Xiamen University from July 2018 to August 2021 were retrospectively analyzed,and 54 patients were treated as the control group during the routine nursing period from July 2018 to January 2020.Fifty-four patients admitted during the implementation of evidence-based concept of operating room infection care from February 2020 to August 2021 were set as the observation group.The length of stay,qualified rate of infection detection in operating room,total incidence of infection and total nursing satisfaction were compared between the two groups.Results The length of hospitalization in the observation group was (14.82±2.53) d,shorter than that in the control group of (19.24±3.72) d,and the difference was statistically significant (P＜0.05).There was no significant difference in the qualified rate of air and sterile bags in the operating room between the two groups (P＞0.05).The qualified rates of hand,disinfectant and object surface detection in the observation group were 98.15%,94.44% and 92.59%,respectively,higher than 83.33%,77.78% and 75.93% of the control group,and the differences were statistically significant (P＜0.05).The total incidence of infection in the observation group was 7.41%,lower than that in the control group(22.22%),and the difference was statistically significant (P＜0.05).The total nursing satisfaction of the observation group was 98.15%,higher than that of the control group (83.33%),and the difference was statistically significant (P＜0.05).Conclusion The operating room infection nursing based on evidence-based concept can effectively improve the qualified rate of infection detection in the operating room,reduce the infection rate,and obtain higher nursing satisfaction.
