医护一体化护理模式对腹腔镜胆囊切除术患者术后康复效果的影响

摘要目的探讨医护一体化护理模式对腹腔镜胆囊切除术（LC）患者术后康复效果的影响。方法选取2018年1月～2019年12月我院收治的150 例LC 患者，按随机数字表法分为两组，每组各75 例。对照组给予常规护理，观察组采用医护一体化护理模式。比较两组术后康复情况、疼痛、并发症发生率和患者配合度及护理满意度。结果观察组的排气时间为（23.53±3.52）h、排便时间为（40.51±4.57）h、肠鸣音恢复时间为（8.98±1.30）h、住院时间为（5.86±1.52）d，均短于对照组的（27.54±4.18）、（45.74±4.29）、（10.56±1.17）h、（7.11±1.34）d，差异有统计学意义（P＜0.05）；两组术后第1 天的疼痛评分比较，差异无统计学意义（P＞0.05）；观察组出院时的疼痛评分为（2.63±0.57）分，低于对照组的（3.44±0.49）分，差异有统计学意义（P＜0.05）；观察组的并发症总发生率为2.67%，低于对照组的12.00%，观察组配合度为93.33%、护理总满意度为94.67%，高于对照组的82.67%、81.33%，差异均有统计学意义（P＜0.05）。结论医护一体化护理模式能加快LC 患者术后康复进程，缩短术后下床、排气等恢复时间，减轻患者术后疼痛，预防并发症的发生，提高患者诊疗配合度和护理满意度，值得推广应用。

Abstract：Objective To explore the effect of integrated nursing mode on the rehabilitation of patients after laparoscopic cholecystectomy (LC). Methods From January 2018 to December 2019, 150 patients with LC in our hospital were selected and divided into two groups according to the random number table method, 75 cases in each group. The control group was given routine nursing, and the observation group was given integrated nursing mode. The postoperative recovery, pain, complication rate, patients′ cooperation degree and nursing satisfaction were compared between the two groups. Results The exhaust time, defecation time, bowel sound recovery time and hospitalization time in the observation group were (23.53 ±3.52), (40.51±4.57) , (8.98±1.30) h and (5.86±1.52) d, which were shorter than those in the control group of (27.54±4.18), (45.74±4.29), (10.56±1.17) h, (7.11±1.34) d, with statistical differences (P＜0.05).There was no difference in pain score between the two groups on the first day after operation (P＞0.05). The pain score of the observation group was (2.63±0.57) points, lower than that of the control group of (3.44±0.49) points, the difference was statistically significant (P＜0.05). The total complication rate in the observation group was 2.67%, which was lower than 12.00% in the control group, the degree of coordination was 93.33% and the total satisfaction with nursing was 94.67% in the observation group, which were higher than 82.67% and 81.33% in the control group, the differences were statistically significant (P＜0.05). Conclusion The integrated nursing mode of medical and nursing can speed up the rehabilitation process of patients after LC shorten the recovery time of getting out of bed and exhausting after operation, reduce the postoperative pain of patients, prevent the occurrence of complications, improve the cooperation degree of diagnosis and treatment and nursing satisfaction of patients, which is worth popularizing and applying.
