手部操锻炼在桃红四物汤加减熏洗治疗类风湿关节炎患者护理中的应用

摘要目的探讨手部操锻炼在桃红四物汤加减熏洗治疗类风湿关节炎（RA）患者护理中的应用效果。方法选取2018年6月～2019年12月我院收治的60例RA患者作为研究对象，采用随机数字表法分为对照组（n=30）和观察组（n=30）。对照组采用桃红四物汤加减熏洗联合常规护理治疗，观察组采用桃红四物汤加减熏洗联合手部操锻炼，比较两组的晨僵时间、关节活动度、疼痛程度、日常生活动能力及护理满意度。结果干预前，两组的晨僵时间、关节活动度、视觉模拟量表（VAS）评分比较，差异无统计学意义（P＞0.05）；干预后，观察组的晨僵时间短于对照组，VAS评分低于对照组、关节活动度高于对照组，差异均有统计学意义（P<0.05）；同组干预后的晨僵时间短于干预前，关节活动度、VAS评分低于干预前，差异有统计学意义（P<0.05）。干预前，两组的Barthel指数（BI）评分、护理满意度评分比较，差异无统计学意义（P＞0.05）；干预后，观察组的BI指数评分、护理满意度评分高于对照组，差异有统计学意义（P＜0.05），两组干预后的BI指数评分、护理满意度评分高于干预前，差异均有统计学意义（P<0.05）。结论手部操锻炼在桃红四物汤加减熏洗治疗RA患者护理的应用效果较好，可改善患者关节症状，缩短晨僵时间，缓解疼痛程度，提高患者日常生活活动能力及护理满意度。

Abstract： Objective To investigate the application effect of hand exercise in nursing of patients with rheumatoid arthritis (RA) treated by modified Taohong Siwu Decoction fumigation and washing.Methods A total of 60 patients with RA who were admitted to our hospital from June 2018 to December 2019 were selected as the research subjects,and they were divided into control group (n=30) and observation group (n=30) according to random number table method.The control group was given modified Taohong Siwu decoction fumigation and washing combined with routine nursing,and observation group was given modified Taohong Siwu Decoction fumigation and washing combined with hand exercise.The morning stiffness time,joint range of motion,pain Degree,activities of daily living and nursing satisfaction were compared between two groups.Results There were no statistical differences in the morning stiffness time,joint range of motion and score of visual analogue scale (VAS) between two groups before intervention (P＞0.05).After intervention,the morning stiffness time of observation group was shorter than that of control group,the VAS score of observation group was lower than that of control group,and the joint range of motion in observation group was higher than that in control group,the differences were statistically significant (P<0.05).After intervention in same group,the morning stiffness time was shorter than that before intervention,and the joint range of motion and VAS score were lower than those before intervention (P<0.05).Before intervention,there were no statistical differences in the scores of Barthel index (BI) and nursing satisfaction between two groups (P＞0.05),and after intervention,the scores of BI and nursing satisfaction in observation group were higher than those in control group,the differences were statistically significant (P<0.05).After intervention,the scores of BI and nursing satisfaction in same group were higher than those before intervention,the differences were statistically significant (P<0.05).Conclusion Hand exercise in nursing of patients with RA treated by modified Taohong Siwu Decoction fumigation and washing has better application effects,it can improve joint symptoms,shorten morning stiffness time,relieve pain degree,and increase activities of daily living and nursing satisfaction.
