Causes and countermeasures of delayed treatment of testicular torsion

Abstract:Objective To investigate the causes of delayed treatment of testicular torsion and to propose solutions.Methods The clinical data of 29 patients with testicular torsion who underwent orchiectomy due to delayed treatment and admitted to the Department of Urology of Fujian Provincial Hospital, Fujian Provincial Jinshan Hospital and Fuzhou Second Hospital from July 2010 to July 2020 were retrospectively analyzed. The average age of the patients was(16.86±2.07) years, ages 12-21 years. There were 21 cases on the left side and 8 cases on the right side, and 29 cases of delayed treatment were analyzed and summarized. Results All the 29 cases were treated with orchiectomy and the other side orchiopexy because of testicular exploration. The main causes of delayed treatment were lack of cognition in 17 cases (58.6%), misdiagnosis and missed diagnosis in 9 cases (31.0%), atypical symptoms in 2 cases (6.9%), false negative of color Doppler ultrasound results in 1 case (3.4%). Conclusion The main reasons for delayed treatment of testicular torsion are the lack of cognition, the lack of experience of the first doctor leading to missed diagnosis and misdiagnosis, atypical symptoms and false negative of color Doppler ultrasound results. The main countermeasures are to attach importance to education and strengthen publicity to improve the awareness of testicular torsion, to improve the professional level and experience of doctors, encourage the development of relevant scientific research, support the grass-roots to cooperate with the government to establish a sound medical system, to improve the early stage of patients with testicular torsion by implementing scrotal physical examination and scrotal color Doppler ultrasound in a short time to improve the early diagnosis rate of testicular torsion.
