以危机管理为基础的护理干预对肛肠手术患者的作用及心理状态的影响

摘要目的分析以危机管理为基础的护理干预对肛肠手术患者的作用及心理状态的影响。方法选取2019年1～12月沈阳市肛肠医院收治的106例肛肠手术患者为研究对象，按随机数表法将其分为对照组和观察组，每组各53例。对照组进行常规护理，观察组进行以危机管理为基础护理干预，比较两组的护理效果。结果观察组护理后的抑郁自评量表（SDS）、焦虑自评量表（SAS）评分低于对照组，差异有统计学意义（P<0.05）；观察组术后6、12 h 的视觉模拟评分（VAS）低于对照组，差异有统计学意义（P<0.05）；两组均出现局部水肿、尿潴留、感染等并发症，但观察组的并发症总发生率为7.55%，低于对照组的26.42%，差异有统计学意义（P<0.05）；观察组的护理总满意度为100.00%，高于对照组的83.02%，差异有统计学意义（P<0.05）。结论以危机处理为基础的护理干预可显著改善患者的焦虑和抑郁状态，降低患者术后疼痛度，降低患者术后并发症发生率，提高患者护理的满意度，实用性强，可在临床推荐应用。

Abstract：Objective To analyze the effect of crisis management-based nursing intervention on function and psychological status of patients undergoing anorectal surgery.Methods A total of 106 patients with anorectal surgery treated in Shenyang Anorectal Hospital from January to December 2019 were selected as the subjects,which were divided into control group and observation group according to random number table method,53 cases in each group.Among them,the control group carried out routine nursing,and the observation group carried out nursing intervention based on crisis management,and the nursing effect of the two groups was compared..Results The self-rating depression scale (SDS),self-rating anxiety scale (SAS) scores in the observation group were lower than those in the control group,the differences were statistically significant (P<0.05).The visual analogue score (VAS) scores at 6,12 h after operation in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).Both groups had local edema,urinary retention,infection and other complications,but the total incidence of complications in the observation group was 7.55%,which was lower than that in the control group of 26.42%,the difference was statistically significant (P<0.05).In the observation group,the total satisfaction rate was 100.00%,which was higher than that in the control group of 83.02%,the difference was statistically significant (P<0.05).Conclusion Nursing intervention based on crisis management can improve the anxiety and depression status of patients,reduce the degree of postoperative pain,reduce the incidence of postoperative complications,improve the satisfaction of patients with nursing,and has strong practicability,which can be widely recommended for clinical application.
