大蒜素在长期卧床老年肺炎患者治疗中的效果分析

摘要目的评估大蒜素对长期卧床老年肺炎患者的临床效果。方法采用前瞻性随机对照研究方法，选取2018年10月～2020年2月合肥市滨湖医院收治的105例长期卧床老年肺炎患者作为研究对象，按照简单随机法分为对照组（n=60）与观察组（n=45）。对照组采用肺炎标准治疗方法，观察组在对照组的基础上加用疗程14 d 的大蒜素治疗。比较两组的病原菌分布情况、炎性指标、临床治疗效果及抗生素使用时间，并记录大蒜素使用的不良反应发生情况。结果 ①常见细菌主要为铜绿假单胞菌（36.2%）、肺炎克雷伯菌（23.8%）、大肠埃希菌（7.6%）、鲍曼不动杆菌（4.8%）、金黄色葡萄球菌（4.8%）。②观察组治疗后的白细胞（WBC）、C-反应蛋白（CRP）水平低于对照组，差异有统计学意义（P<0.01）。③观察组的总有效率为97.8%；对照组的总有效率为76.7%，观察组的总有效率高于对照组，差异有统计学意义（P<0.05）。④观察组的抗生素使用时间短于对照组，差异有统计学意义（P<0.01）。⑤观察组发生不良反应2例，其中腹泻1例，皮疹1例，发生率为4.44%。结论长期卧床老年肺炎患者在常规治疗基础上加用大蒜素治疗后能显著缩短抗生素使用时间，且不良反应发生率低，可在临床推广应用。

Abstract：Objective To evaluate the clinical effect of Allicin in the treatment of long-term bedridden senile pneumonia.Methods A prospective randomized control study was conducted.Totally 105 long-term bedridden senile pneumonia patients treated in Hefei Binhu Hospital from October 2018 to February 2020 were selected,and were divided into the control groups (n=60) and the treatment group (n=45) with simple randomization.The control group was treated with pneumonic standard treatment,and the treatment group was given extra Allicin for 14 d on the basis of the therapy of the control group.Distribution of pathogenic bacteria,inflammation indicators,therapeutic efficacy and durations of antibiotics were compared and analyzed between the two groups.Allicin adverse reactions and its incidence rate were recorded throughout the course..Results ①The most common pathogens were Pseudomonas aeruginosa (36.2%),Klebsiella pneumonia (23.8%),Escherichia coli (7.6%),Acinetobacter baumannii (4.8%) and Staphylococcus aureus (4.8%)in long-term bedridden senile pneumonic patients.②The levels of white blood cell (WBC) and C-reactive protein(CRP) of the treatment group were lower than those in the control group after therapy,the differences were statistically significant (P<0.01).③The rate of therapeutic effect of 97.8% in the treatment group was higher than 76.7% of the control group,the difference was statistically significant (P<0.05).④The antibiotics using time was shorter than that in the control group,the difference was statistically significant (P<0.01).⑤The incidence rate of adverse reactions in the treatment group was 4.44%,one was diarrhea,and the other was skin rash.Conclusion Additional Allicin treatment in long-term bedridden senile pneumonic patients can significantly shorten the use of antibiotics and improve the curative effection on the basis of treatment with antibiotic,and it could be safe with low incidence of adverse reactions when used in clinic.
