临床急救措施在产后出血伴凝血功能障碍中的应用效果

摘要目的针对产后出血伴凝血功能障碍者采取临床急救措施，分析其对患者疗效、凝血功能及并发症发生情况的影响。方法选取2016年2月～2019年12月赣州市立医院收治的100例产后出血伴凝血功能障碍患者，采用随机数字表法分为对照组和观察组，每组各50例。对照组进行盆腔动脉栓塞术治疗，观察组在盆腔动脉栓塞术治疗基础上行急救措施。对比分析两组患者的病情恢复、凝血功能及术后并发症的发生情况。结果观察组患者的病情恢复总有效率（94.00%）高于对照组（74.00%），差异有统计学意义（P＜0.05）；术前两组的凝血酶原时间（PT）、活化部分凝血活酶时间（APTT）、纤维蛋白原（FIB）及凝血酶时间（TT）比较，差异无统计学意义（P＞0.05）；两组患者术后的PT 均长于术前，而术后的APTT 和TT 均短于术前，术后的FIB水平均低于术前，差异有统计学意义（P＜0.05）；术后观察组的PT 长于对照组，APTT 和TT 短于对照组，FIB水平低于对照组，差异有统计学意义（P＜0.05）；观察组患者的并发症总发生率[6.00%（3/50）]低于对照组[22.00%（11/50）]，差异有统计学意义（P＜0.05）。结论针对产后出血伴凝血功能障碍患者采用临床急救措施，不仅可有效改善患者的凝血功能，而且降低患者的并发症总发生率，促进病情恢复。

Abstract：Objective To analyze the effect of clinical first aid measures on the curative effect,coagulation function and the incidence of complications taken for patients with postpartum hemorrhage and coagulation disorders.Methods A total of 100 patients with postpartum hemorrhage and coagulation disorders who were admitted to Ganzhou Municipal Hospital from February 2016 to December 2019 were divided into control group and observation group according to random number table method,50 cases in each group.The control group was treated with pelvic arterial embolization,and the observation group was given first aid measures on the basis of the control group.The recovery of condition,coagulation function and postoperative complications were compared between two groups..Results The total effective rate of recovery in the observation group (94.00%) was higher than that in the control group (74.00%) the difference was statistically significant (P<0.05).There were no significant differences in prothrombin time (PT),activated partial thromboplastin time (APTT),fibrinogen (FIB) and thrombin time (TT) between the two groups before operation (P>0.05).The PT in the two groups after operation was longer than that before operation,while the APTT and TT after operation were shorter than those before operation,and the levels of FIB after operation were lower than those before operation,the difference was statistically significant (P<0.05).After the operation,the PT in the observation group was longer than that in the control group,the APTT and TT were shorter than those in the control group,and the level of FIB was lower than that in the control group,the differences were statistically significant (P<0.05).The total incidence of complications in the observation group (6.00% [3/50]) was lower than that in the control group (22.00% [11/50]),the difference was statistically significant (P<0.05).Conclusion Clinical first aid measures taken for patients with postpartum hemorrhage and coagulation disorders can not only improve the coagulation function of patients,but also reduce the incidence of complications and promote the recovery of the disease.
