应急强化护理在心内科重症监护室急性心肌梗死患者中的应用

摘要目的探讨应急强化护理在心内科重症监护室急性心肌梗死（AMI）患者中的临床应用价值。方法选择2019年2月～2020年4月沈阳医学院附属第二医院收治的80例AMI 住心内科重症监护室患者，按照数字随机法分为两组，每组各40例。对照组进行常规护理，观察组联合应急强化护理，比较两组干预前后肌钙蛋白I（Tn-I）水平；比较两组住院期间的并发症发生情况；比较两组干预后疾病自我管理能力和生活质量情况。结果干预后两组的Tn-I 水平均低于干预前，差异有统计学意义（P＜0.05），且观察组的Tn-I 水平低于对照组，差异有统计学意义（P＜0.05），住院期间观察组发生心力衰竭、室壁瘤、危及生命心律失常的总比例低于对照组，差异有统计学意义（P＜0.05），观察组的疾病自我管理能力评分高于对照组，差异有统计学意义（P＜0.05），生活质量评分高于对照组，差异有统计学意义（P＜0.05）。结论针对心内科重症监护室患者实施应急强化护理，可有效减少患者心功能损伤，减少并发症发生率，提高患者对疾病的主观认知，从而改善预后。

Abstract：Objective To explore the clinical application value of emergency intensive care with acute myocardial infarction (AMI) in intensive care unit of cardiology department.Methods A total of 80 cases with AMI in the intensive care unit of cardiology department the Second Affiliated Hospital of Shenyang Medical College from February 2019 to April 2020 were selected and divided into two group according to the method of random number table,40 cases in each group.The control group was received routine nursing,the observation group was received emergency intensive nursing.Then the changes of cardiac troponin I (Tn-I) level before and after the intervention were compared between the two groups.The incidence of complications during hospitalization was compared between the two groups.The disease selfmanagement ability and life quality after intervention were compared between the two groups.Results After the intervention,the level of Tn-I in both groups was lower than that before intervention,the difference was statistically significant (P＜0.05),the level of Tn-I in the observation group was lower than that of the control group,the difference was statistically significant (P＜0.05).The total proportion of heart failure,ventricular aneurysm and life-threatening arrhythmia in the observation group during hospitalization was lower than that of the control group,the difference was statistically significant (P＜0.05).The score of disease self-management ability in the observation group was higher than that of the control group,the difference was statistically significant (P＜0.05).The score of life quality was higher than that of the control group,the difference was statistically significant (P＜0.05).Conclusion Emergency intensive care for patients in intensive care unit of cardiology department can effectively reduce the cardiac function damage of patients,reduce the incidence of complications,improve the subjective cognition of patients to diseases,and thus improve the prognosis.
