Abstract：Objective To investigate the clinical effect of controlled low central venous pressure technique in the treatment of hepatectomy for intrahepatic bile duct stones.Methods A total of 86 patients with intrahepatic bile duct calculi treated by hepatectomy who admitted to Affiliated Jiujiang Hospital of Nanchang University were selected and divided into the control group and the observation group according to random number table method,with 43 cases in each group.The control group was administrated by intravenous infusion with conventional methods,and the observation group was administrated by intravenous infusion with controlled low central venous technique.The amount of intraoperative blood loss,blood transfusion,proportion of blood transfusion,postoperative plasma urea nitrogen (BUN),creatinine changes (Cr),infusion volume before liver resection,and hemodynamic indexes were compared between the two groups.Results The amount of intraoperative blood loss and blood transfusion in the observation group were less than those in the control group,the proportion of blood transfusion was less than that in the control group,and the differences were statistically significant (P＜0.05).There were no statistically significant differences in plasma BUN and Cr levels between the two groups of patients before and after surgery (P＞0.05).The infusion volume before liver resection in the observation group was less than that in the control group,the central vein pressure (CVP) was lower than that in the control group,and the differences were statistically significant (P＜0.05).There was no significant difference in heart rate(HR) between the two groups (P＞0.05).Conclusion Patients with intrahepatic bile duct calculi treated by hepatectomy administrated by intravenous infusion with controlled low central venous pressure technique can reduce intraoperative blood loss,blood transfusion and infusion volume,and is beneficial to the postoperative recovery of patients.
