Abstract:Objective To study the guiding value of serum lactate clearance rate and procalcitonin (PCT) in the prognosis of children with severe pneumonia.Methods A retrospective analysis of the data of 70 children with severe pneumonia admitted to the Third People′s Hospital of Huizhou from January 2018 to January 2020 was made,according to the prognosis of the children,the selected children were divided into the survival group (n=56) and the death group (n=14).The pediatric critical illness score (PCIS),serum PCT level and lactate clearance rate of children at 24 hours after admission between the two groups were compared,and the influence of PCT,lactate clearance rate and the interaction between the two on the prognosis of children and the guiding value were analyzed.Results The PCT of the death group was higher than that of the survival group,PCIS and the lactate clearance rate were lower than those of the survival group,and the differences were statistically significant (P<0.05).The interaction of PCT,lactate clearance rate and both of them had an impact on the prognosis of children with severe pneumonia (P<0.05).The PCT had higher guiding value for the prognosis of children with severe pneumonia (AUC>0.9,P<0.05),the critical value of 8.985 ng/mL,the sensitivity was 78.57%,and the specificity was 96.43%.The lactate clearance rate and the fitting had moderate guidance value for the prognosis of children with severe pneumonia (0.7<AUC≤0.9,P<0.05).The critical value of lactate clearance rate was 27.375%,the sensitivity was 91.07%,and the specificity was 78.57%;the fitting sensitivity of the two was 85.71%,and the specificity was 80.36%.Conclusion The serum lactate clearance rate and the PCT level have a definite effect on the prognosis of children with severe pneumonia and have moderate guiding value.
