Abstract:Objective To investigate the effect of continuous increase of blood lactic acid duration (TLac>2 mmol/L) on prognosis of severe patients.Methods Retrospective analysis on 114 severe patients with acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ) score >15 points and blood lactic acid (Lac) >2 mmol/L in Lanzhou First People′s Hospital was made from January 2015 to December 2018.Their blood gas and Lac levels were measured and recorded every 6 hours from admission to hospital,until Lac levels fell to normal range or patient died.The general situation,APACHE Ⅱscore,TLac>2mmol/L and prognosis were recorded and analyzed.Patients were divided into survival group (62 cases) and death group (52 cases) according to the final outcome.The general condition,Lac,APACHE Ⅱscore at admission,and TLac>2mmol/L were compared between the two groups.Cox regression analysis was performed on the TLac>2mmol/L and outcome of all subjects to evaluate the effect of TLac>2mmol/L on prognosis of severe patients.Results There were no statistically significant differences between the two groups in gender,age,and first Lac and APACHE Ⅱscore of patients at admission (P>0.05).The TLac>2mmol/L in the death group was higher than that in the survival group,and the difference was statistically significant (P<0.05).Cox regression analysis indicated that the longer TLac>2mmol/L was,the lower the survival rate of patients and the higher the risk of death.Conclusion An transient increase of Lac may not have an effect on the prognosis of severe patients,but the TLac>2mmol/L has a significant effect on the prognosis of severe patients.The longer the TLac>2 mmol/L,the lower the survival rate and the higher the risk of death.
