Clinical efficacy of bioelectric technology in the perioperative period of gynecological total pelvic floor reconstruction based on enhanced recovery after surgery

摘要目的 探讨基于加速康复外科理念(ERAS)在妇科全盆底重建围手术期中应用仿生物电技术的临床疗效。方法 选取2020年5月至2021年8月广东省东莞市厚街医院收治的60 例妇科全盆底重建围手术期患者作为研究对象,采用随机数字表法将其分对照组与观察组,每组各30 例。对照组患者接受ERAS 干预,观察组患者在对照组的基础上接受仿生物电技术干预。比较两组患者的术后肠鸣音恢复状况、围手术期指标、术后疼痛程度、并发症。结果 观察组患者术后12、24、48 h 时的规律肠鸣音出现率高于对照组,且术后12、24、48 h 时的肠鸣音次数多于对照组,差异有统计学意义(P<0.05)。两组患者的手术时间、术中出血量比较,差异无统计学意义(P>0.05)。观察组患者的术后首次排便时间、术后首次排气时间、首次进食时间短于对照组,差异有统计学意义(P<0.05)。观察组患者术后12、24、48 h 时的视觉模拟评分法(VAS)评分低于对照组,差异有统计学意义(P<0.05)。观察组患者的并发症总发生率(10.00%)低于对照组(33.33%),差异有统计学意义(P<0.05)。结论 妇科全盆底重建围手术期患者在接受ERAS 理念干预的基础上,再实施仿生物电技术干预,可显著缓解患者术后疼痛症状,减少并发症,促进术后恢复。
Abstract:Objective To evaluate the efficacy of biomimetic technology in perioperative period of gynecological total pelvic floor reconstruction based on enhanced recovery after surgery (ERAS).Methods A total of 60 patients in the perioperative period of gynecological whole basin reconstruction treated from May 2020 to August 2021 in Houjie Hospital of Gongguan City,Guangdong Province were selected as the research object.They were divided into control group and observation group by random number table method,with 30 cases in each group.The control group received ERAS concept intervention,and the observation group received bioelectric technology intervention on the basis of the control group.The postoperative bowel sound recovery,perioperative indexes,postoperative pain and complications were compared between the two groups.Results The occurrence rate of regular bowel sounds at 12,24 and 48 h in the observation group were higher than those in the control group,and the number of bowel sounds at 12,24 and 48 h were more than those in the control group,the differences were statistically significant (P<0.05).There were no statistically significant differences in operation time and intraoperative bleeding between the two groups (P>0.05).The first defecation time,first exhaust time and first eating time in the observation group were shorter than those in the control group,the differences were statistically significant (P<0.05).The visual analogue scale (VAS) scores in the observation group were lower than those in the control group at 12,24 and 48 h after operation,the differences were statistically significant (P<0.05).The incidence of complications in the observation group (10%) was lower than that in the control group (33.33%),and the difference was statistically significant (P<0.05).Conclusion Perioperative patients with gynecological total pelvic floor reconstruction can significantly alleviate postoperative pain symptoms,reduce complications and promote postoperative recovery on the basis of eras intervention.
