Diagnostic value of modified Alvarado score combined with CT multiplane reconstruction in children with acute appendicitis

Abstract:Objective To investigate the value of modified Alvarado score combined with CT multiplanar reconstruction(MPR) in the diagnosis of acute appendicitis in children. Methods A total of 116 children with suspected acute appendicitis who were treated in the Department of General Surgery of the Affiliated Hospital of Jinggangshan University from January 2019 to September 2021 were analyzed retrospectively. All children underwent modified Alvarado score and MPR. Postoperative pathological diagnosis results were taken as the gold standard, the sensitivity, specificity, positive predictive value and negative predictive value of different diagnostic methods were statistically analyzed. Results Among 116 children with suspected acute appendicitis, 109 cases were diagnosed as acute appendicitis by postoperative pathology. The sensitivity, specificity, positive predictive value and negative predictive value of the combined diagnosis of the diagnostic methods were higher than Alvarado scoring system and MPR diagnosis,and the differences were statistically significant (P<0.017). Conclusion Modified Alvarado score combined with MPR had higher sensitivity, specificity, positive predictive value and negative predictive value in the diagnosis of acute appendicitis in children. Therefore,it has the value of popularization and application in clinical work.
