脊柱结核诊断及鉴别诊断的研究进展

摘要脊柱结核是由结核分枝杆菌引起的最常见的肺外结核和骨关节结核。其发病隐匿，早期临床症状和体征不明显，与脊柱其他感染性疾病相似，难以明确诊断，常导致漏诊、误诊，甚至误治。患者不能得到及时诊疗，随着病情加重，常在出现神经损伤、骨质破坏、脊柱不稳、后凸畸形等并发症后就诊，给临床治疗增加难度，也严重影响患者工作及生活质量。“早诊断、早治疗”是影响脊柱结核预后的关键因素。随着诊断技术快速发展和治疗方案不断更新，其诊疗水平显著提升，但受耐药、传染病传播等因素的影响，近年来脊柱结核患者数量反呈上升趋势。为提高临床医师对脊柱结核的诊断水平，本文就近年来国内外对脊柱结核典型临床症状体征、辅助检查及鉴别诊断研究进展进行综述。

Abstract： Spinal tuberculosis is the most common extrapulmonary and osteoarticular tuberculosis caused by Mycobacterium tuberculosis. Its onset is hidden, early clinical symptoms and signs are not obvious, similar to other infectious diseases of the spine, it is difficult to diagnose, often lead to missed diagnosis, misdiagnosis, and even mistreatment. Patients cannot be diagnosed and treated in time. With the aggravation of the disease, they often visit the doctor when nerve injury, bone destruction, spinal instability, kyphosis and other complications, which increases the difficulty of clinical treatment and seriously affects the quality of work and life of patients. Early diagnosis and treatment are the key factors affecting the prognosis of spinal tuberculosis. With the rapid development of diagnostic technology and the continuous updating of treatment regimens, the level of diagnosis and treatment has improved significantly. However, the number of patients with spinal tuberculosis has increased in recent years due to drug resistance, infectious disease transmission and other factors. In order to improve the diagnostic level of clinicians for spinal tuberculosis, this paper reviewed the research progress of typical clinical symptoms and signs, auxiliary examination and differential diagnosis of spinal tuberculosis in recent years.
