憩室化瘀汤治疗子宫切口憩室致血瘀型经期延长的临床效果

摘要目的观察憩室化瘀汤治疗子宫切口憩室致血瘀型经期延长的临床效果。方法选取2018年9月～2019年10月于湖南中医药大学附属岳阳医院妇产科门诊就诊的84 例子宫切口憩室致血瘀型经期延长的患者作为研究对象，按照随机数字表法将其分为对照组和治疗组，每组各42 例。对照组患者口服去氧孕烯炔雌醇片治疗，治疗组患者在对照组的基础上加用憩室化瘀汤患者。两组均治疗3 个月经周期后，比较两组患者治疗前后的行经时间、子宫切口憩室大小及中医症候积分，并观察两组患者的治疗效果及不良反应发生情况。结果两组患者治疗后的行经时间均短于本组治疗前，差异有统计学意义（P＜0.05）。两组患者治疗后的子宫切口憩室宽度及深度均小于本组治疗前，差异有统计学意义（P＜0.05）。两组患者治疗后的中医证候积分均低于治疗前，差异有统计学意义（P＜0.05）。治疗组患者治疗后的行经时间、子宫切口憩室宽度及深度及中医症候积分改善情况均优于对照组，差异有统计学意义（P＜0.05）。治疗组患者的治疗总有效率高于对照组，差异有统计学意义（P＜0.05）。两组均无不良反应发生。结论憩室化瘀汤治疗子宫切口憩室致血瘀型经期延长具有明显的效果，且与单纯口服去氧孕烯炔雌醇片相比更能改善临床症状，减轻患者痛苦，值得大力推广。

Abstract：Objective To observe the clinical effect of Qishi Huayu Decoction in the treatment of prolonged menstrual period in the blood stasis pattern caused by uterine incision diverticulum. Methods Totally 84 patients with prolonged menstrual period out of blood stasis caused by uterine incision diverticulum who were treated in the Clinic of Obstetrics and Gynecology, Yueyang Hospital Affiliated to Hu′nan University of Chinese Medicine were selected as the research objects. They were divided into the treatment group and the control group according to the random number table method, 42 cases in each group. The control group took Desogestrel Ethinyl Estradiol Tablets, and the treatment group added Qishi Huayu Decoction on the basis of the control group. After three menstrual cycles of treatment, the time of menstruation, the size of uterine incision diverticulum and the score of traditional Chinese medicine (TCM) syndrome before and after treatment were compared between the two groups, and the treatment effect and incidence of adverse reactions of patients were observed in the two groups. Results The time of menstruation in both groups after treatment was shorter than that before treatment, and the difference was statistically significant (P＜0.05). The width and depth of uterine incision diverticulum after treatment in both groups was smaller than that before treatment, with statistical significance (P＜0.05). The TCM syndrome scores of the two groups after treatment were lower than those before treatment,and the differences were statistically significant (P＜0.05). The improvement of the menstruation time, width and depth of uterine incision diverticulum and TCM syndrome score after treatment in the treatment group were better than those in the control group, and the differences were statistically significant (P＜0.05). The total effective rate of treatment in the treatment group was higher than that in the control group, and the difference was statistically significant (P＜0.05). No adverse reactions occurred in the two groups. Conclusion Qishi Huayu Decoction has obvious curative effect in the treatment of prolonged menstrual period in a blood stasis pattern caused by uterine incision diverticulum. Compared with oral administration of Desogestrel Ethinyl Estradiol Tablets alone, it can better alleviate clinical symptoms and patients′ pain, which is worthy of promotion.
