新生儿呼吸道合胞病毒不同亚型的研究进展

摘要呼吸道合胞病毒（RSV）是新生儿呼吸道感染的常见病原体，其临床表现缺乏特异性，易被忽略导致重症感染，且新生儿由于免疫系统尚未发育完全，其感染后的严重程度较年长儿严重，更易发生爆发感染。根据RSV 的G 蛋白与单克隆抗体反应的不同以及G 蛋白基因的不同，将RSV 分为A、B 两种亚型，由于不同亚型RSV 的G蛋白中和抗体不能提供有效的亚型间交叉保护作用，故可反复多次感染。关于两种亚型临床表现，疾病严重程度是否具有差异，国内外存在诸多报道，但结论不一。针对RSV 基因型与疾病的相关性，本文对新生儿呼吸道合胞病毒不同亚型流行病学、临床表现、治疗及预防进行综述，旨在进一步为疫苗的研制及临床特异性防治方案的制定提供科学依据。

Abstract：Respiratory syncytial virus (RSV) is the common cause of respiratory infections in neonate. The clinical manifestations are lack of the specificity, therefore it can be ignored to cause severe infection. Moreover, the neonatal immune system is not fully developed, the severity of disease is more serious than older children and more prone to explosive infection. It has been classified into two genotypes of A and B (RSV-A and RSV-B) based on the difference response of RSV G protein with monoclonal antibodies and G protein genes, and multiple infections can be repeated because the neutralizing antibodies of different subtypes of RSV do not provide effective pritection in different genotypes.There are many reports at home and abroad on whether the disease severity is different between the two subtypes, but the conclusions are different. To investigate the relationship between the different genotypes and disease, this paper reviews the epidemiology, clinical manifestations, treatment and prevention of neonatal respiratory syncytial virus, aiming to provide a scientific basis for vaccine development and the formulation of clinical specific prevention.
