基于护理主导医患参与的区域性防治在静脉血栓栓塞症预警管理中的效果

摘要目的分析基于护理主导医患参与的区域性防治在静脉血栓栓塞症预警管理中的应用效果。方法选取2018年3月至2019年7月佛山市南海区人民医院收治的66 例住院患者作为常规组，选取2019年8月至2020年12月收治的66 例住院患者作为研究组。常规组患者采用常规护理干预，研究组患者在常规的基础上采用基于护理主导医患参与的区域性防治。比较两组患者的深静脉血栓、肺静脉血栓发生率；比较两组患者干预前后的D-二聚体（D-D）水平、血液流变学[血浆黏度（PV）、纤维蛋白原（FIB）]；比较两组患者干预前后的疾病知识掌握情况。结果研究组患者的深静脉血栓发生率为3.03%（2/66）、肺静脉血栓发生率为3.03%（2/66），均低于常规组的12.12%（8/66）、13.64%（9/66），差异有统计学意义（P<0.05）；干预后，研究组患者的D-D 水平低于常规组，差异有统计学意义（P<0.05）；干预后，研究组患者的PV、FIB 水平均低于常规组，差异有统计学意义（P<0.05）；干预后，研究组患者的疾病知识掌握评分高于常规组，差异有统计学意义（P<0.05）。结论基于护理主导医患参与的区域性防治应用于静脉血栓栓塞症预警管理中，可提高患者的疾病知识掌握度，改善血液高凝，促进血液循环，降低静脉血栓栓塞症发生率，临床推广价值较高。

Abstract：Objective To analyze the application effect of regional prevention and treatment based on nurse-led doctor-patient participation in early warning and management of venous thromboembolism. Methods A total of 66 inpatients admitted to Foshan Nanhai People′s Hospital from March 2018 to July 2019 were selected as the routine group, and 66 inpatients admitted to Foshan Nanhai People′s Hospital from August 2019 to December 2020 were selected as the research group. The routine group received routine nursing intervention. The team adopted a regional approach based on nurse-led doctor-patient participation on a routine basis. The incidence of deep vein thrombosis and pulmonary vein thrombosis was compared between the two groups. D-dimer (D-D) level and hemorheology (plasma viscosity [PV], fibrinogen [FIB]) were compared between the two groups before and after intervention. The knowledge of disease was compared between the two groups before and after intervention. Results The incidence rate of deep vein thrombosis (3.03%, 2/66) and the incidence rate of pulmonary vein thrombosis (3.03%, 2/66) in the study group, were both lower than 12.12% (8/66) and 13.64% (9/66) in the conventional group, the differences were statistically significant(P<0.05). After intervention, the D-D level of the study group was lower than that of the conventional group, and the difference was statistically significant (P<0.05). After intervention, PV and FIB levels in the study group were lower than those in the conventional group, the differences were statistically significant (P<0.05). After intervention, the disease knowledge mastery score in the study group was higher than that in the conventional group, and the difference was statistically significant (P<0.05). Conclusion The regional prevention and treatment based on nurse-led doctorpatient participation can improve the knowledge of the disease, improve blood hypercoagulability, promote blood circulation,reduce the incidence of venous thromboembolism in the early warning management of venous thromboembolism,and has high clinical promotion value.
